External Beam Radiation Therapy (EBRT) vs Brachytherapy
Modern Concepts in Radiation Oncology
Radiation therapy remains one of the cornerstone modalities in cancer treatment. It can be broadly classified into:
1️⃣ External Beam Radiation Therapy (EBRT)
A non-invasive technique where high-energy photons, electrons, or protons are delivered from outside the body toward the tumor target.
Key Characteristics:
• Treatment delivered using linear accelerators (LINACs)
• Advanced planning with CT/MRI/PET fusion
• Techniques include:
3D-CRT
IMRT
VMAT
IGRT
SBRT/SRS
Proton Therapy
Advantages:
✔ Excellent coverage for large or deep tumors
✔ Ability to treat regional lymphatics
✔ Highly conformal dose distribution with modern planning systems
✔ Image guidance improves precision and reduces toxicity
Limitations:
• Increased dose exposure to surrounding normal tissues
• Longer overall treatment duration in many protocols
• Organ motion and setup uncertainties may affect accuracy
2️⃣ Brachytherapy (Internal Radiation Therapy)
A highly specialized form of radiation therapy where radioactive sources are placed directly inside or adjacent to the tumor.
Types:
• Intracavitary
• Interstitial
• Intraluminal
• Surface mold brachytherapy
Dose Rate Classifications:
LDR (Low Dose Rate)
HDR (High Dose Rate)
PDR (Pulsed Dose Rate)
Common Radioisotopes:
• Iridium-192
• Cesium-137
• Iodine-125
• Palladium-103
Why Brachytherapy Is Unique
Brachytherapy allows: ✔ Very high tumor dose escalation
✔ Rapid dose fall-off sparing adjacent organs at risk (OARs)
✔ Short overall treatment time
✔ Excellent local control in selected malignancies
It remains essential in the management of: • Cervical cancer

• Esophageal and bronchial palliation

Image-Guided Adaptive Brachytherapy (IGABT)
Now considered standard of care in locally advanced cervical cancer.
Current Recommendations:
• MRI-guided target delineation preferred
• Adaptive planning based on tumor regression
• Combined EBRT + concurrent chemotherapy + HDR brachytherapy

• HR-CTV D90 optimization
• Strict OAR constraints for:
Bladder

Sigmoid
Small bowel
Emerging Technologies:
