Case Presentation:
A 36-year-old female patient presented with chronic abdominal distress/pain, persistent dyspepsia, and recurrent vomiting.
Endoscopic Findings (Upper Endoscopy):
• Lesion & Extension: A circumferential, infiltrative lesion was noted extending from 43 cm to 56 cm from the oral cavity, involving the gastric body down to the proximal antrum.
• Gastric Compliance: The gastric cavity was rigid, non-distensible, and failed to insufflate easily (Leather Bottle Appearance), with an absolute absence of normal peristalsis.
• Mucosal Pattern: Severely thickened and fixed mucosal folds. The mucosa exhibited a distinct Waffle-like appearance / Leopard skin pattern due to the underlying submucosal infiltration.
• Ulcerations: 1. A large neoplastic-looking ulcer on the anterior surface with a necrotic floor.
2. Another large ulcer on the posterior surface with a necrotic floor.
• Intervention: Multiple targeted biopsies were obtained from the edges of both ulcers, the necrotic tissue, and the leopard skin mucosa (submitted for histopathological evaluation).
• Passage: The scope successfully passed through the rigid lumen down to the antrum.
Impression:
The endoscopic picture and mucosal architecture (clearly enhanced via NBI/WLE) are highly suggestive of Gastric Linitis Plastica (Diffuse Gastric Infiltration). Awaiting histopathological confirmation.

مريضة تبلغ من العمر 36 عاماً، كانت تعاني من آلام مستمرة في البطن، عسر هضم شديد، وقلس (قيء) متكرر. بعد الفحص الطليعي، قررنا إجراء منظار علوي للجهاز الهضمي لمعرفة السبب الحقيقي وراء هذه الأعراض الطويلة.
ماذا أظهر الفحص؟
أثناء الفحص، تبين وجود تغلظ شديد وجدار صلب غير مرن في المعدة، وهو ما يُعرف طبياً بمظهر "زجاجة الجلد" أو (Leather bottle appearance)، حيث تفقد المعدة قدرتها الطبيعية على التمدد والحركة الدودية نتيجة ارتشاح تحت مخاطي عميق. المخاطية ظهرت بنمط يشبه "جلد النمر" أو وافل البسكويت (Waffle-like appearance) مع وجود قرحتين كبيرتين بنسيج يبدو متحوراً (Neoplastic).
تم أخذ عينات متعددة (Biopsies) من حواف القرح، النسيج النخري، ومن جدار المعدة المرتشح لإرسالها للتحليل الباثولوجي للتأكد النهائي. الحالة من الناحية الإكلينيكية والمنظارية تشير بقوة إلى (Linitis Plastica).
💡 الرسالة من الحالة: أعراض مثل عسر الهضم أو القيء المستمر لا يجب إهمالها أو الاكتفاء بالمسكنات معها. التشخيص المبكر بالمنظار هو المفتاح دائماً للتعامل الصحيح.
