Sometimes the most challenging cases are not the most severe ones… they are the ones where treatment options must be carefully adapted to the patient's age and overall condition.
This patient is 80 years old.
When she first came to us, the swelling had already begun to affect her comfort, mobility, and daily activities. Normally, more intensive compression strategies might be considered, but in her case we had to balance effectiveness with safety and tolerance.
Our challenge was clear:
✔️ Use lighter compression techniques
✔️ Avoid excessive pressure that she might not tolerate
✔️ Achieve meaningful reduction as quickly as possible
✔️ Maintain comfort and compliance throughout treatment
Despite these limitations, the progress achieved in approximately two weeks has been very encouraging.
Cases like this remind us that successful lymphedema treatment is not about applying the strongest compression possible. It is about finding the right compression for the right patient at the right time.
In elderly patients, comfort, safety, skin condition, mobility, and quality of life are just as important as volume reduction itself.
I am very pleased with the progress we have seen so far, but even more pleased that we were able to achieve it without overwhelming the patient or compromising her ability to continue treatment.
The journey is still ongoing, and we hope to see even greater improvement in the coming weeks.
Because age may change the treatment plan, but it should never take away the opportunity for improvement.

I’m truly honored and grateful to share that my latest case report has now been officially published in the Limb Preservation Journal.
As an Egyptian and African Physical Therapist and Certified Lymphedema Therapist, it means a lot to me to contribute — even in a small way — to respected international scientific publications that focus on limb preservation, wound care, lymphedema, and patient-centered rehabilitation.
Before anything, I thank God for continuously blessing the small efforts we make every day, multiplying them into opportunities to learn, teach, help patients, and connect with incredible professionals around the world. Every patient story teaches us something deeper about resilience, dignity, and hope.
This publication highlights a case of advanced primary lower-limb lymphedema where conservative physical therapy–led management helped avoid amputation and restore patient engagement in care. Beyond the physical improvements, the case also emphasizes the importance of psychological support, patient education, and long-term therapeutic partnership.
I would like to sincerely thank the editorial and review team at the Limb Preservation Journal for their professionalism, guidance, and support throughout the publication process. I truly hope this will be the beginning of continued collaboration in the future.
My biggest hope is that efforts like these can contribute to increasing awareness about:
Lymphedema
Lipedema
Chronic wounds
Limb preservation
Post-surgical rehabilitation
Compression therapy
And the critical role of conservative physical therapy in improving quality of life
There is still a long road ahead for awareness and education in our region and globally, but step by step, together, we move forward.

Determination often becomes the strongest part of the treatment plan.
This patient came to us at a very advanced age, with a history of a previous stroke and long-standing lower limb swelling.
Clinically, cases like this are never simple. Age, circulation challenges, and past medical events can make progress slower and more delicate.
But what stood out the most was her determination.
Despite the physical challenges, she showed remarkable commitment throughout the sessions.
Every appointment, every step of the treatment, she arrived with the same mindset: keep trying and keep improving.
That level of persistence became a powerful motivation for me as a therapist.
It pushed me to give my absolute best in every session, aiming to achieve the safest and best possible outcome for her.
The improvement you see here is not only the result of therapy —
it is also the result of a patient who refused to let age or past illness define what was still possible.
Cases like this remind us that sometimes the most important factor in recovery is not the protocol…
but the patient’s will to move forward.
And for that, she deserves the greatest credit.

Some cases carry a quiet kind of honor.
This patient is special to me for reasons that go far beyond the clinical outcome.
Years ago, he was treated by someone I have always considered one of my true role models in this field. When that therapist emigrated and was no longer available, the patient chose to stop treatment altogether.
Not because he didn’t believe in therapy — but because trust, once built, is not easily replaced.
For many years, he refused to continue care.
Recently, his son—who lives abroad and his friend who works in the same specialty—recommended that he come to me.
According to him, I was the only option that felt familiar, close or maybe better to the standard of care his father once trusted.

This case represents a form of gratitude I don’t talk about often.
Gratitude toward those I have always looked up to.
Those who shaped my understanding of this profession.
Those whose paths I try, every day, to honor by doing my work with the same integrity and respect.
Progress will continue, and the journey is still ongoing.
But moments like this remind me why role models matter — and why success is not about replacing them, but about carrying their values forward in your own way.

This case represents advanced lipedema that progressed into secondary lymphedema in a patient over 70 years old, with significant weight and severe mobility limitations.
What you see here was achieved after only three CDT sessions.
Not a final result — but an important message.
For this patient, improvement is possible if commitment is maintained.
Reduced heaviness, better limb contour, and improved movement are often the first steps before visible long-term change.
And yes, the journey is still ongoing.
Cases like this are the reason I wrote 📚"The Silent Battle of Lipedema".
Chapter 10 is dedicated entirely to real clinical journeys like this one — to offer hope for patients and practical, realistic guidance for clinicians managing complex, late-stage, high-risk cases.
Lipedema is not just “fat.”
And meaningful improvement doesn’t always start with dramatic change — sometimes it starts with the first three steps forward.
