Trauma may disrupt the anatomy but careful reconstruction can preserve it.

This case presented with a traumatic cataract associated with iridodialysis, iris sphincter ruptures, and zonular weakness.

With the capsular bag preserved and supported by a capsular tension segment and capsular tension segment , an IOL was successfully implanted in the bag. Pupilloplasty with pupillary cerclage was then performed to restore a rounder, more functional pupil.

Preserve the bag. Stabilize the lens. Reconstruct the pupil.
