● عن طريق ملاحظة إرتفاع الانزيمات

● هتشوف مين فيهم ارتفاعه ملحوظ (مع العلم ان التلاتة هيرتفعو معاك.)
1️⃣ pattern
➖️ ALT / AST: ↑↑↑ Predominant elevation
➖️ ALP Mild–moderate
● Total bilirubin ↑↑
● Direct bilirubin: Often ↑
● هنا الارتفاع الملحوظ كان فى ال ALT/AST

2️⃣ pattern
➖️ ALP: ↑ ↑↑↑ Predominant elevation
➖️ ALT / AST: Mild–moderate ↑
● Total bilirubin ↑↑
● Direct bilirubin : Usually ↑↑
● هنا الارتفاع الملحوظ كان فى ال ALP

2️⃣ :
🔬 The — approach

= (ALT / ALT ULN) ÷ (ALP / ALP ULN)

➖️ R ≥ 5 → Hepatocellular
➖️ R ≤ 2 → Cholestatic
➖️ R > 2 and < 5 → Mixed pattern

:
➖️ ALT = 600 U/L, ULN = 40 → 15× ULN
➖️ ALP = 180 U/L, ULN = 120 → 1.5× ULN
✅️ = 15 ÷ 1.5 = 10 → pattern

:
➖️ ALT = 120 U/L / 40 = 3× ULN
➖️ ALP = 600 U/L / 120 = 5× ULN
✅️ = 3 ÷ 5 = 0.6 → pattern

⚠️ Important: Bilirubin tells you that jaundice is present, but the pattern of ALT/AST versus ALP elevation is much more useful for distinguishing hepatocellular from cholestatic injury.
