The patient presented with marked unilateral facial swelling. Intraorally, there was a large ulceronecrotic lesion in the posterior maxilla, with ill-defined bone destruction on the panoramic radiograph.
What would be at the top of your differential diagnosis?
1)Invasive fungal infection
2)Squamous cell carcinoma
3)Hematolymphoid malignancy
4) Mucoepidermoid carcinoma

Jaw swellings are a common presentation with different causes but their clinical appearance doesn’t always reflect their nature.
This was a painless, gradually increasing swelling causing obvious facial asymmetry.
Histology showed abundant myxoid stroma with scattered spindle and stellate cells.
Diagnosis: Odontogenic myxoma.
A benign odontogenic tumor, but locally aggressive and known for its tendency to recur.

Gingival swellings are very common, and many of them look quite similar clinically. Most turn out to be reactive lesions, but occasionally histopathology gives us a less common diagnosis.
This was a small, painless gingival swelling that clinically could easily be mistaken for a fibroma.
Histology showed a localized myxoid connective tissue with scattered spindle and stellate cells.
Diagnosis: Oral focal mucinosis.
A rare benign lesion and a good reminder that not every gingival swelling is what it seems clinically.

الحالتين اللي في الصور دول بيأكدوا إن دور طبيب الأسنان مش بس يعالج الأسنان... لكن يفحص الفم كله و يتخذ قرار امتي يحول الحالة لمتخصص و امتي يكتفي فقط بالمتابعة
الحالة الأولى كانت جاية تعمل زراعة أسنان. قبل ما يبدأ أي إجراء، الطبيب عمل فحص كامل للفم، ولاحظ بقعة شكلها مش مطمن. نصح المريض ياخد عينة، وبالفعل اتاخدت وكانت النتيجة Squamous Cell Carcinoma (SCC). الاكتشاف كان بدري، وده فرق جدًا في خطة العلاج.
أما الحالة التانية، فالطبيب شاف برضه تغيرات مقلقة، وشرح للمريض إن أخذ عينة هو الطريقة الوحيدة نعرف التشخيص الحقيقي. لكن الخوف خلاه يرفض ويأجل.
وللأسف... لما رجع بعد فترة، الورم كان كبر بشكل واضح، وبقى العلاج أصعب وأعقد من لو كان اتشخص من البداية.
الرسالة هنا مش إننا نخوف الناس... الرسالة إننا منستهونش بأي حاجة شكلها مش طبيعي داخل الفم.
فحص الفم الكامل لازم يكون جزء أساسي من كل كشف أسنان، وأي قرحة أو بقعة أو تغيير مستمر لأكتر من أسبوعين يستحق يتفحص كويس، ولو الطبيب طلب عينة، فده علشان يطمن أو يشخص بدقة، مش علشان يقلق المريض.
دقايق زيادة في الكشف... ممكن تنقذ سنين من المعاناة.
والتشخيص المبكر دايمًا هو أقوى خطوة في العلاج.
