🫀Prominent Crista Terminalis – A Normal Anatomical Variant

A prominent crista terminalis is a muscular ridge in the right atrium extending from the superior vena cava (SVC) to the inferior vena cava (IVC). When prominent, it may appear as an intracardiac mass on echocardiography.

🔍 Echo Features
✅ Smooth, well-defined linear or crescent-shaped structure
✅ Located along the lateral wall of the right atrium near the SVC–RA junction
✅ Homogeneous echogenicity, similar to myocardium
✅ Non-mobile or minimally mobile
✅ No independent blood flow or stalk

📌 Differential Diagnosis
• Right atrial thrombus
• Myxoma or other cardiac tumors
• Chiari network
• Eustachian valve
• Vegetation

💡 Clinical Pearl
Recognition of a prominent crista terminalis prevents unnecessary investigations and interventions. If uncertainty persists, TEE, contrast echocardiography, cardiac CT, or cardiac MRI can help confirm the diagnosis.

Take-home message: A prominent crista terminalis is usually a benign normal variant—not a pathological mass.
