More than 20 years ago, a boy around eleven years old, who attended an international private school, visited my clinic. He suffered from cones dystrophy—a stable medical condition that does not deteriorate over time unless vision rehabilitation and eye usage are neglected. I began rehabilitation sessions and introduced optical aids to him. However, just a few days later, I was shocked by a phone call from his panic mother, declaring that her son had lost his sight completely!
I asked her to bring him in immediately. While speaking with her at the clinic, I noticed the boy walking toward the examination area with confident steps and no hesitation, which made me doubt the reality of total blindness. I re-examined him and tried every possible method, but he insisted on repeating a single phrase: "I can't see," accompanied by a subtle smile—unusual for someone who has suddenly lost their sight.
I explained to his mother that the issue might have a psychological dimension and referred her to a colleague who is a professor of child psychiatry. The psychological assessment revealed no clear psychiatric disorder. I then suggested to my colleague exposing him to a "reality check" to resolve this claim gently, without confrontation, and out of compassion for his parents.
We agreed to advise the parents to take him on a tour of a public school for the blind, presenting it as the only remaining educational option following his "total sight loss." Indeed, the boy toured the classrooms with his parents, and just a few hours later that evening, I received an overjoyed call from the mother: her son had "regained" his sight as soon as they returned home!
Years after this incident, I had the honor of co-supervising a Master's thesis at the Faculty of Medicine, Ain Shams University, alongside esteemed professors. The study addressed:
"Neuropsychological Changes and the Impact of Health Status on Quality of Life in Visually Impaired Children"
The study involved 50 children, divided into two groups:
• Group 1: 30 children with visual impairment.
• Group 2: 20 healthy children (control/comparison group).
Assessment Parameters:
1. Evaluation of visual functions and visual perception.
2. IQ testing and determination of mental and cognitive age.
3. Assessment using anxiety and depression scales.
4. Evaluation of the impact of general health status on "quality of life."
Results and Recommendations
o Children with visual impairment exhibited high levels of frustration and depression, along with a marked decline in quality of life.
o IQ test results showed lower rates of intelligence, mental age, and cognitive age in the visually impaired group compared to the control group.
• Recommendations:
o Early Intervention: The critical need to start visual and psychological rehabilitation immediately upon diagnosis within the first months of life.
o Community and Educational Awareness: Raising awareness of these conditions and appropriate handling strategies among teachers and educators, ensuring children can continue their visual, psychological, and social rehabilitation within the school environment.
