1️⃣ : using *Type I anti-CD* *20* monoclonal Ab ( this reminds us about using Type II anti-CD 20 Obinutuzumab )

2️⃣
====> 2️⃣ND Assessment check point , at 24 months , revealed that Ritux was Significantly Superior to Cyclosporine in achieving Remission ..

==> Remission was obtained in 2️⃣ arms , Clinical ( Proteinuria) and Immunological ( AntiPLA2R-Ab) as well..

3️⃣ ==> All patients had a 3️⃣ months of receiving RAS blockade ..

4️⃣==> All patients had a stable kidney Function , Creatinine clearance of 4️⃣0️⃣ ml/ min ..

5️⃣===> All patients had a Bx Proven Primary Membranous nephropathy with heavy Proteinuria of 5️⃣ gm/ day ..

6️⃣ ===> 6️⃣0️⃣% of patients in the arm of Rituximab achieved Remission at 12 months and 24 months

👉🏻60 % at 12 months = non inferior to Cyclosporine ...

👉🏻 60 % at 24 months >> Superior to Cyclosporine that achieved 20 % ..

When you measure serum IgA in patient with ESM (Episodic Synpharyngetic Macroscopic) Hematuria to Diagnose IgA Nephropathy..! though it is a Histopathological diagnosis 🔬

📌 Note that Serum IgA is increased in only 1/3 of cases of IgA Nephropathy..

📌 The pathogenesis of IgA Nephropathy is about changes in the Physiochemical properties or structure of IgA , rather than an ABSOLUTE increase in amount of IgA ..
