2.7 DO, 2.6 DOP w/Deep margin elevation + vital pulp therapy (VPT).

(2.6) The tooth was restored with LC RMGI and composite resin using an open sandwich technique. Indirect pulp capping was done using calcium hydroxide (Dycal). This restoration will suffice in the interim while we wait to see how the tooth responds to the VPT.

Definitely among the more challenging cases I've come across so far. The decay extended subgingival and made for a difficult experience getting a good seal and marginal adaptation. The caries progression was quite severe (ICDAS-6) so I utilized a selective carries removal technique to prevent unnecessary pulp exposure.

I've been finding it challenging to contour anatomy in the posterior maxilla (indirect). Hoping it gets easier as I get more reps.

Fortunately the tooth was asymptomatic at a 4week follow-up and remains so after 60 days. Staying optimistic!

Simple occlusal composite w/buccal pit
Tooth #19/3.6

________
-patient had limited mouth opening but other than that very simple case
-Occlusal restored using Nexcomp (Nano-hybrid resin) (A2 shade)
-Buccal pit restored w/ Admira Fusion x-tra (Nano-hybrid resin) (universal shade)

________
Lessons learned/discussion:
-try a different composite brand. not very happy with the shade matching of the universal-shade composite (but it could be my technique at fault)
-work on composite placement technique: better anatomy, shade selection, maybe buy a brush for better adaptation
-work on consistency in photographs. the pics have different perspectives and magnification
-consider experimenting with stains/tints for more realistic composites

I'm happy with the outcome (and so was the patient). However, going forward, I would like to work on improving the aesthetics of my composite restorations

Posterior composite restoration
#18/3.7 MO

___________

Very straightforward class II case.
-removed carries, obtained clean margins
-placed rubber dam (W8A clamp on 3.7, floss ligature on 3.5)
-selective etch technique + universal bond system
-proximal wall/box was restored using flowable and Nano-hybrid resin (snowplow technique)
-rest of cavity restored with Admira Fusion x-tra

___________
Lessons learned/discussion:
-proper band selection: select larger matrix band next time
-work on speed: due to time constraints finishing/polishing was rushed
-rubber dam: stretch out the dam more to avoid bunching of material
