حضرت إلى العيادة مريضة شابة تشكو من ألم شديد متكرر في أعلى البطن منذ نحو ثلاثة أشهر، يزداد بشكل واضح بعد تناول الطعام ولا يستجيب للعلاج الدوائي المعتاد. خضعت المريضة لعدة فحوصات معملية وإشعاعية لم تُظهر سببًا عضويًا واضحًا، مما أدى إلى تشخيصها مبدئيًا كحالة اكتئاب شديد مصحوب بقلق مزمن.
بإعادة تقييم الحالة وأخذ تاريخ مرضي دقيق، تبين أن الألم مصحوب بفقدان وزن ملحوظ خلال الفترة الأخيرة، بالإضافة إلى غثيان متكرر. كما أفادت المريضة بأن شدة الألم تقل نسبيًا عند الانحناء للأمام، ولا يوجد تاريخ مرضي آخر يُذكر.
في ضوء هذه المعطيات، تم الاشتباه في تشخيص نادر وهو متلازمة الرباط المقوس المتوسط (Median Arcuate Ligament Syndrome - MALS)، الناتجة عن ضغط الرباط على الشريان الزلاقي، مما يؤدي إلى أعراض نقص التروية الحشوية خاصة بعد تناول الطعام
young female patient presented to the clinic with a three-month history of severe, recurrent epigastric pain. The pain was notably postprandial and did not respond to conventional medical therapy. She had previously undergone multiple laboratory and radiological investigations, all of which failed to identify an organic cause. Consequently, she was initially diagnosed with severe depression associated with chronic anxiety.
Upon careful re-evaluation and detailed history taking, several important red flag symptoms were identified. The pain was associated with significant weight loss over the recent period, as well as recurrent nausea. The patient also reported that the pain was partially relieved by leaning forward, a clinically relevant observation. There was no other significant past medical history.
In light of these findings—particularly the presence of postprandial pain, weight loss, and positional relief—a rare diagnosis was suspected: Median Arcuate Ligament Syndrome (MALS). This condition results from compression of the celiac artery by the median arcuate ligament, leading to visceral ischemia, especially after meals.
