Un gran grupo de terapeutas de mano con ganas de aprender y profundizar en el conocimiento de la cirugía de la mano. Alumnos de distintas provincias de España, con una ilusión increíble por aprender y progresar en la terapia de mano. Me siento muy afortunado de ser parte de este programa. Paciente de 30 años de edad, diagnosticado a los 10 años de una encondromatosis múltiple de la mano.

A los 20 años de edad, tras objetivarse un aumento significativo del tamaño de las lesiones y una importante destrucción ósea, se realizó una resección completa del cuarto radio de la mano.

Once años después de la intervención, el paciente presenta una estabilidad radiológica de las lesiones, sin evidencia de progresión significativa, asociada a un excelente resultado funcional.

Actualmente realiza actividad deportiva de forma habitual y mantiene una vida completamente normal, sin limitaciones funcionales relevantes.

Este caso pone de manifiesto que, en determinados pacientes con encondromatosis múltiple de la mano y afectación extensa de un radio, una resección completa del radio afectado puede proporcionar estabilidad de la enfermedad y excelentes resultados funcionales a largo plazo, permitiendo al paciente recuperar una actividad cotidiana y deportiva prácticamente normal. A 30-year-old patient was diagnosed at the age of 10 with multiple enchondromatosis of the hand, compatible with Ollier disease.

At the age of 20, following a significant increase in the size of the lesions and substantial bone destruction, a complete resection of the fourth ray of the hand was performed.

Eleven years after surgery, the patient presents with radiological stability of the lesions, with no evidence of significant progression, and an excellent functional outcome.

The patient currently participates in sports regularly and leads a completely normal life, with no relevant functional limitations.

This case demonstrates that, in selected patients with multiple enchondromatosis of the hand and extensive involvement of a single ray, complete resection of the affected ray can provide long-term disease stability and excellent functional outcomes, allowing the patient to return to a virtually normal daily and sporting life.

Prótesis de interfalángica proximal mediante técnica WALANT

Paciente de 70 años con artrosis avanzada de la articulación interfalángica proximal del 4.º dedo, intervenida mediante técnica WALANT (Wide Awake Local Anesthesia No Tourniquet).

Se realizó la implantación de una prótesis KeriFlex, permitiendo comprobar intraoperatoriamente la movilidad y estabilidad de la articulación.

El resultado ha sido espectacular: paciente muy contenta y satisfecha, con ausencia de dolor y una excelente recuperación funcional.

Un caso que demuestra las posibilidades de la cirugía de mano con técnicas mínimamente invasivas y anestesia WALANT, buscando no solo aliviar el dolor, sino también preservar y recuperar la función de la mano. Proximal interphalangeal joint arthroplasty using the WALANT technique

A 70-year-old patient with advanced osteoarthritis of the proximal interphalangeal joint of the ring finger underwent surgery using the WALANT (Wide Awake Local Anesthesia No Tourniquet) technique.

A KeriFlex prosthesis was implanted, allowing intraoperative assessment of joint stability and range of motion.

The outcome has been spectacular: the patient is extremely happy and satisfied, with no pain and excellent functional recovery.

A great example of the possibilities offered by modern hand surgery using minimally invasive techniques and WALANT anesthesia, with the goal of not only relieving pain but also preserving and restoring hand function. Fractura-luxación radiocarpiana secundaria a fractura intraarticular de radio distal

Se trata de una fractura intraarticular de radio distal con comportamiento biomecánico de fractura-luxación radiocarpiana, asociada a lesiones importantes de las estructuras estabilizadoras de la muñeca.

Se realiza tratamiento asistido por artroscopia, mediante reducción y reconstrucción anatómica de la superficie articular, asociada a estabilización percutánea de la fractura del radio.
Durante la artroscopia se objetiva, además, desinserción radial del complejo fibrocartílago triangular (CFCT), así como desinserción de la cápsula dorsal en toda su extensión, lesiones que contribuyen de forma significativa a la inestabilidad radiocarpiana.
La artroscopia de muñeca resulta fundamental en este tipo de lesiones, ya que permite una visualización directa de la superficie articular, confirmar y optimizar la reducción de los fragmentos intraarticulares y detectar y valorar lesiones ligamentarias y capsulares asociadas que pueden pasar inadvertidas mediante el control fluoroscópico convencional.
De este modo, el tratamiento permite abordar de forma integral tanto la reconstrucción anatómica de la superficie articular como las lesiones de las estructuras estabilizadoras radiocarpianas, favoreciendo una recuperación funcional adecuada y disminuyendo el riesgo de inestabilidad residual y alteraciones degenerativas secundarias. Patient with knive injury at the dorsal aspect of forearm. Drop of third fourth and fifth finger with no extensor tendon function of this fingers. Normal wrist extension. Always think more than muscle injury. In this case a partial PIN injury associated with muscle tear was found. Micro surgical repair was needed.
