I was honored to be invited by my Dr. L. Mike Nayak Dr Dominic Bray to contribute an article to Facial Plastic Surgery Clinics of North America Journal on my experience and surgical philosophy in lower blepharoplasty.

A major part of my career has been dedicated to periorbital surgery. Over the years, one principle has become increasingly clear: we must distinguish between the operation that is easier for the surgeon and the operation that is anatomically most appropriate for the patient. Lower blepharoplasty should not be defined only by the incision, but by the deformities that must be corrected and the adjunctive maneuvers required to achieve a safe, stable, and harmonious result.

In appropriately selected patients, I strongly favor the transcutaneous approach. Although technically more demanding, it provides greater exposure, versatility, and anatomical control. It allows the surgeon to address skin excess, orbicularis management, fat modification or repositioning, lower eyelid support, canthal stabilization, and refinement of the lid–cheek junction, and cheek suspension within the same operation.

The transconjunctival approach remains valuable in selected patients, particularly when isolated fat prolapse is present without significant skin excess, laxity, instability, or lid–cheek deformity or feestoon. However, it has limitations because it does not permit the full range of corrective and supportive procedures(examples: malar feestoon correction or tear valley deformity…ects)

The transcutaneous approach can overcome these limitations and treat almost all components of lower eyelid aging. The trade-off is a longer recovery, especially when adjunctive procedures are required. Nevertheless, because it can address skin, muscle, fat, support, and contour comprehensively, I consider it the most versatile approach for almost all patients requiring lower eyelid rejuvenation.

Ultimately, the best operation should be selected for the patient, not for the convenience of the surgeon.

24 DAYS POST FACELIFT + NECKLIFT (DEEP PLANE TECHNIQUE) + TEMPORAL LIFT

The deep plane facelift is preferred because it releases the retaining ligaments and elevates the skin and underlying SMAS as a single anatomical unit, rather than relying on skin tension or muscle alone. This approach restores facial volume and repositions descended tissues to their original, youthful location, producing more durable results. By avoiding excessive skin traction, it reduces the risk of a pulled or operated appearance. It is particularly effective in correcting the jawline, and neck while preserving facial expression. I am very happy to share this beautiful result 1 year after facial rejuvenation surgery. In her case, we performed a Deep Plane Facelift and Neck Lift, combined with a Temporal Lift, creating a comprehensive and harmonious rejuvenation of the face and neck.

Modern facial rejuvenation is not simply about tightening the skin. The Deep Plane Facelift allows us to release and reposition the deeper facial tissues, restoring youthful contours while preserving the patient’s natural identity. The Neck Lift complements this by improving the jawline and cervical contour. A lo largo de mi trayectoria como cirujano y médico estético, he aprendido que cuidar va mucho más allá de un tratamiento. Significa escuchar, acompañar y estar al lado de las personas en los momentos que más lo necesitan.

Este último año, la vida me ha permitido vivir ese aprendizaje desde otra perspectiva, acompañando a mi mujer durante un cáncer de mama. Una experiencia que me ha recordado que el bienestar no solo depende de la salud, sino también del apoyo, la empatía y de sentir que no estás solo.

Por eso, este año estaré junto al equipo de FACEXPERT en el evento de la Asociación Española Contra el Cáncer de Marbella .malaga.
