Yesterday, I got an early morning call for consultation. A 38 years old young man arrived in our hospital at midnight after having 4 episodes of massive Bloody vomiting over a day. He came from northwest zone of Tigrai. On arrival he was hypotensive and anemic(HGB 6.8g/dldespite low BP, a hemoconcentrated hemoglobin, normal platelet count). Abdominal ultrasound was normal(with special focus on the liver, portal venous system and spleen).
While trasfusing him with whole blood, we did an Emergency Endoscopy. It revealed a single protuberant vascular lesion at the gastric fundus, with hemolyzed blood inside the gastric cavity.No esophageal varices. We considered a rare vascular lesion-Dieulafoy lesion. We did Band ligation. Bleeding controlled, patient stabilized. Emergency endoscopy is a life saving procedure.
