The case here is an immediate implant molar:
- STRAUMANN TE RN
- for lower molar compatible for RN, 2mm GH.
SSA is prepared extraorally and layered with primer and composite by my assistant .
After implant is placed, the SSA GF (pre layered with composite) is inserted.
Amazingly, socket is sealed already at 95%. 😍
Customization took less than 5 min.

SURGICAL EXTRACTION BURR KIT:
Minimally invasive extraction is THE key of success in immediate implant.
Most of the challenges and errors that can occur from an immediate implant surgery comes from this part.
It is from far the most unpredictable part of the whole process from:
✅ Timing perspective
✅ Technical perspective
✅ Ergonomics perspective.

It can happen in 10 min 😁 or more than an hour🥵…
it can be a traumatic experience for the patient 🤯.

🎩So few tips here that helps (not miracle):
- Remove prosthetic structure 1 by 1 before working on the roots
- Always separates roots on pluri rooted tooth (or almost always)
- Try to avoid elevating flaps or tearing buccal tissue
- manipulate and elevate the lingual part if possible
- Use a surgical burr kit for every extraction process
Many times I have been asked the references and sequences of my surgical burr kit. Here is the detail of the kit that I systematically have on my surgical set up and the illustration of the steps on a typical case.

This has made the difference for me … BIG BIG TIME.

Low traumatic extraction… …
This can make you struggle more than usual…
This can make you exhausted more than after a running session…
This can make you feel your are wasting your time…
But at the end you don’t regret it because it s the only way to conduct immediate implant placement in good conditions …
Objectives:
- preserving bone, tissue
- placing implant the same day
- Reducing post op
- saving overall treatment time
- being more profitable with no biological compromise

Way to succeed:
-plan adequate time for surgery
- separate root every time
- Get a Atraumatic bur kit
- Equipped yourself with great instruments (I use Hufriedy extraction cassette, Luxator )
- get trained on immediate treatment workshops
