Etiologies are classically categorised into immune-mediated, drug-induced, neoplastic and idiopathic disorders. Medical history, physical examination, autoimmune testing, chest CT and bronchoalveolar lavage support a standardised diagnostic work-up. Several patterns can inform the diagnosis, but they are not disease-specific.

https://zurl.co/78hCF The MICROINHALO trial included 250 critically ill intubated patients across 10 ICUs in Italy and Israel. Automatic management did not significantly reduce Day 3 bacterial tracheobronchial colonization compared with manual management. Clinically diagnosed and microbiologically confirmed VAP rates were lower with automatic management. It also showed fewer cuff-pressure values outside the safety range and a higher daily subglottic secretion drainage volume.

https://zurl.co/EVYam A multinational ICU cohort included 845 adults with microbiologically confirmed bacterial infections who received empirical antimicrobials. Appropriate therapy was given to 87.7% of patients and was associated with lower ICU mortality, more antimicrobial-free days and more mechanical ventilation-free days. The survival benefit was consistent across diagnostic certainty and most pronounced at SOFA scores of 3 to 9.

The 36th ADQI meeting developed multidisciplinary international expert recommendations across five working groups. Priorities included AKI risk, fluid management, indications for CRRT, CRRT delivery during ECMO, biomarkers, blood purification and drug dosing. Knowledge gaps and research priorities were identified.

Their role may lie in supporting dynamic assessment of the injured brain through indirect indicators of tolerance to ICP burden. Multimodal non-invasive monitoring may help detect deterioration, reduced compensatory reserve, impaired adaptation, harmful trajectories and early decompensation while informing escalation strategies.
