Changes in compliance, dead space and cardiac output can help distinguish recruitment from harmful overdistension. Gas exchange should therefore be interpreted alongside markers of perfusion and oxygen transport when ventilator settings are adjusted.

https://zurl.co/ekgqC A bag-valve mask with a positive end-expiratory pressure valve is a reasonable alternative to a non-rebreather mask when noninvasive ventilation is unavailable. Its pressure effect is unreliable in real prehospital conditions and requires a leak-free seal, sufficient gas flow and often a second operator. Noninvasive ventilation remains the best-supported preoxygenation strategy for reducing peri-intubation hypoxemia when feasible and tolerated.

The protocol reduced tidal de-recruitment and carbon dioxide levels compared with baseline pressure control ventilation, but increased driving pressure, mechanical power and dynamic strain. Further optimisation is needed to limit overdistension and hemodynamic compromise.

Brain stress can involve hypoxemia, hemodynamic instability, inflammation and metabolic derangements. Standardized delirium screening, judicious sedative use and mitigation of immobility, sleep disruption and sensory impairment are targeted strategies, but variable definitions, surveillance practices and outcome measures limit precise estimates.

The apparent benefit was linked mainly to fewer mild-to-moderate postoperative acute kidney injuries, while other components did not significantly differ. Reporting serum creatinine and urine output separately, with cumulative hypotension exposure, could clarify the findings.
