As we come into the summer, I just want to let all my lovely patients know that I see you, I care about you, and I value you

I know many women are feeling quite desperate when they come to see me

They have often been coping with unpleasant, uncomfortable, distressing symptoms for a long time, and feel helpless and hopeless about their situation

Please know that I care about your wellbeing, and I am passionate about helping as many women as I can

I also need to make sure I look after myself and have downtime with my family so that I can be my best, most present self for you in my clinic

So, I am taking some time off here and there over the summer

Clinic will be mostly open, but I will be working reduced hours

Thank you for your patience and understanding, and I look forward to seeing you at your next appointment ๐Ÿงก

Do you get stress incontinence when you exercise?

(Stress incontinence is bladder leakage caused by an increase in intra-abdominal pressure - e.g. with exercise, coughing, sneezing etc)

Please know - you are definitely not alone; this is actually very common

If this happens to you, your instinct might be to think you need to work on your pelvic floor exercises. But that's not always the right treatment...

Sometimes you have to continue doing the very exercise that causes the leaking, but adapted slightly to let your body strengthen up your pelvic floor muscles

Here's what I mean by that...

Say a woman comes to me having issues with running causing stress incontinence.

I will often get them to do lots of running drills, but keep all the running drills below the threshold of having a leak

That way, we can gradually train your continent threshold

When they run within the threshold they can manage without leaking, their subconscious pelvic floor contraction is spot on, doing exactly what it needs to do

We need to expand the range and build up the muscles so they can subconsciously work under more stress ๐Ÿ’ช

Sometimes I will get women to work on single leg strength and that can make all the differenceโ€ฆ

Pelvic floor muscle training and pregnancy...

Ladies who've had a baby, every time we see our midwife, we know the drill, don't we...

Have you done your pelvic floor exercises? Have you done your pelvic floor exercises, have you done them, have you done them? ๐Ÿ˜…

But it's not always that simple!

๐Ÿ‘‰ Even if women do know to do their pelvic floor exercises, how do they know they're doing them correctly?

๐Ÿ‘‰ How do they know they're working the muscles in the most efficient way?

๐Ÿ‘‰ How do they know if they're activating the right muscles?

๐Ÿ‘‰ What about pelvic floor length work?? (Very important ahead of a vag.inal delievry)

And these are just a few issues; there are more!

I'm not blaming midwives - they can't refer every single pregnant woman to pelvic floor physio (although I think that would be the ideal scenario, but that's a post for another day...๐Ÿ˜ƒ)

So, I guess what I am saying is that there is an alternative...

When it comes to pelvic floor health, having a baby is a significant event that we know places stress on the pelvic floor

We all cope differently with this, depending on how straightforward the birth is, whether instruments were used (or you have a C-Section), how big/small you and your baby are, your general health and pre-pregnancy pelvic floor function, etc...

Some ladies come to me having had birth injuries; some have had a straightforward birth and want reassurance that they are recovering normally

Both are equally important, and receive the treatment and support that's right for them ๐Ÿงก

I feel like everyone who comes through my doors is frazzled these days - work, home, kids, aging parents, responsibilities... just life!

Many of them seem close to burnout.

I have most definitely been feeling a bit burnt out myself lately, too. Have you?

I've been noticing and thinking about this a lot, and I'm going to share some thoughts on this over a few posts coming up soon...

Women, I want to make sure I've got the right balance when you come in and see me

I want to:

Enable you; not disable you
Inform you; not scare you
Advise you; not give you strict rules

I am constantly midful of this. Being a good physio means striking a balance, and I aim for that in every single appointment

If you've seen a physio for pelvic floor issues before, and it didn't resolve the problem, it doesn't mean it won't work...

If you've seen a physio for pelvic floor issues before, and it didn't resolve the problem, it doesn't mean it won't work if you try again with a different physio

That can happen for several reasons, for example:

๐Ÿ‘‰ You didn't click with your physio, so you didn't feel safe and supported

๐Ÿ‘‰ You didn't believe it could work for you, so you held back a bit

๐Ÿ‘‰ You were busy and overwhelmed and couldn't give enough to your treatment.

Things change! You might be in a different place now, and ready to give it another go

Don't be put off if it didn't work for you before

Make sure you ask around for a reputable and recommended specialist physiotherapist, and try again ๐Ÿงก

Not all pelvic floor issues are caused by a weak pelvic floor

They can sometimes be due to *overactive* muscles, where your pelvic floor is constantly braced and can't relax

This causes similar symptoms to a weak pelvic floor, but the treatment is very different!!

The other thing thatโ€™s important to consider is sub-conscious pelvic floor activity. Sometimes I assess women and they can contact and relax their pelvic floor really well but they are still leaking with exercise / impact. In this case, rehab needs to focus on sub-conscious and reflexive pelvic rehab.

See a specialist pelvic physiotherapist for an assessment, and get on track with a treatment plan tailored to your needs ๐Ÿค—

Bladder leaking is a taboo subject.

As a society, we don't really talk about it openly, and as a result, women feel embarrassed and that they are the only ones experiencing leaking, or rushing to the loo all the time.

It is SOOO much more common than you think.

Even in younger women.
Even in women who have never had children.

I am on a mission to talk more openly about pelvic floor issues so that women know how to get the help they deserve ๐Ÿงก
