📢 IMPORTANT CHANGES TO FIT NOTE REQUESTS
We’ve introduced a new Fit Note request process to help us process requests more efficiently and ensure Fit Notes are issued appropriately and in line with NHS guidance.
📝 If you need a Fit Note:
• You must complete a Fit Note Assessment Questionnaire before your request can be reviewed by a clinician.
• Questionnaires are available at reception, by email on request, or via our website https://thepracticeofhealth.nhs.wales/about-us/fit-notes/fit-note-continuation-questionnaire/
• Please make sure all sections are completed — incomplete questionnaires will be returned and your request will not be processed until all information has been provided.
🏥 Recently seen in hospital?
If your absence relates to a hospital admission, hospital procedure, outpatient procedure, or recent treatment from a consultant, your hospital clinician should provide your Fit Note for the recovery period. Please contact the relevant hospital department or consultant’s secretary directly.
ℹ️ Please note: A Fit Note is not automatically issued following a request. All requests are subject to clinical assessment, and our clinicians will consider your duties, whether workplace adjustments could support you, whether amended duties or reduced hours may be appropriate, and any treatment or support you are receiving.
