Our Healthier Neighbourhoods PCN Carers Coffee afternoon takes place every Thursday 1pm-2pm at our fellow PCN practice Heatherlands Medical Centre. A welcoming group, supported by our Care Coordinators, they'll be on hand to provide support, advice and signposting for patients who are carers for others.
