GUESS THE DIAGNOSIS

49 year old male ran the half marathon yesterday despite having knee issues on the lead up to the race. He woke this morning and struggled to straighten his knee. It was extremely painful to put weight through it and was especially painful doing the stairs. He has knee swelling and a catching sensation..

ANSWER...

Meniscus tear.

A meniscus tear is a common knee injury where the rubbery, C-shaped cartilage—acting as a shock absorber between the thigh bone and shin bone—is damaged, causing knee pain, swelling, and locking. It often happens during running when twisting, pivoting, or placing high-impact, repetitive stress on the knee while bearing weight, particularly on uneven surfaces.

In this client's case I think he has a degenerative tear, which are more common in runners over 35-40. The tear can occur as the meniscus naturally loses elasticity with age. Increased mileage or intensity can cause this weakened cartilage to fray or tear without a significant accident.

Next step due to severity of his symptoms is an MRI scan to see if surgery is required.

Back pain is incredibly common, but not all back pain is the same — and that’s why getting a proper physiotherapy assessment matters.

A thorough physio assessment doesn’t just look at where it hurts. It helps identify the underlying cause, guides the right treatment approach, and importantly, determines whether further referral or intervention may be needed.

A lot of people understandably ask for scans when they’re in pain. However, scans aren’t always necessary. In many cases, a detailed clinical assessment combined with monitoring how symptoms respond to treatment can provide a clear and accurate diagnosis.

That said, there are times when further investigation is important.

For example, I recently worked with a patient experiencing back pain alongside nerve symptoms. While physiotherapy helped to some extent, their symptoms didn’t fully resolve as expected. Based on the assessment and their response to treatment, it was clear that further investigation by a specialist surgical team was needed. As you can see from the MRI, the disc was protruding and pressing on the nerve that exited the spine at that level, causing pain and nerve symptoms into the leg.

The outcome? The patient received the appropriate surgery and is now recovering well.

That being said, not every disc prolapse or disc issue requires surgery and can respond really well to physiotherapy treatment.

The key message: start with the right assessment. Physiotherapy plays a crucial role not only in treatment, but in making sure you’re on the right pathway for your recovery.

A huge well done to one of my younger clients whos team won their football league at the weekend.

She came to me with significant instability and pain in her knee, which was preventing her from playing football to her usual high standard. She presented with MCL laxity and and patellar tracking issues and had to step back from her usual training to focus on rehab for a while.

With a guided approach back to the sport that she loves, her team has not only won the league, but she has walked out of clinic tonight with a huge smile on her face and is no doubt a stronger, more resilient player because of it.

Stripping it back to basics with this client who loves running, but who has unfortunately been in a never ending cycle of injuries, rest and return to running, without fully solving the problem.

This time, he decided to get himself assessed properly and after a good M.O.T, it was clear that there were biomechanical issues in the whole of the lower limb. This has resulted in multiple areas of either overloading or underloading and therefore weakness, which in turn has been making him more prone to injuries.

We are therefore focusing on cross training on the bike for now to maintain his cardio fitness whilst we strengthen where needed and correct biomechanical loading and movement patterns. Importantly, we are also adding in strict rest days to allow muscles to repair and regenerate.

This video shows him working hard in clinic and getting ever so closer to getting back to his running routine, but better and stronger than ever before. Trusting the process and seeing the results 💪

Pain, brain and beyond episode 1.

Short and sweet introductory episode for our new podcast which can be accessed via Spotify (link below). We explain who we are, a little bit about our backgrounds in sport and our professions and why we are working collaboratively with athletes and sports people during recovery.

We hope that we can continue to help people who are aiming to return to sport post injury, both physically and mentally, to be at the top of their game with full confidence with both their body and mind.
