🕰️ Ageing gracefully doesn’t mean accepting changes you no longer recognise as “you.”

This lovely patient in her seventies wanted to address the heaviness of her upper eyelids, which had gradually developed over time. Following upper lid blepharoplasty, she looks brighter, more refreshed and less tired – while still looking completely like herself. 👩

The before and after images highlight an important message: it’s never “too late” to consider treatment if something is affecting your confidence or quality of life. Age alone should not be the deciding factor. Careful assessment of overall health, suitability for surgery and realistic expectations are far more important.

It’s also worth noting that we have not yet treated the lower eyelids (yet!). The improvement you can already see is solely from the upper lid procedure, demonstrating just how much excess upper eyelid skin can contribute to an aged or fatigued appearance.

Subtle changes can make a remarkable difference. The goal is not to erase every sign of ageing, but to restore a fresher, more rested appearance that reflects how you feel on the inside.

📸 "Are you sure this is the same patient?"

That was the reaction at our clinic when these photographs were seen!

Our patient had undergone upper lid blepharoplasty and later showed us her before and after images.

Looking at them, you could be forgiven for thinking they were two different people. 👩

Now, before anyone rushes to conclusions, it's worth remembering that photographs can be incredibly deceptive.

Even someone who has never had surgery can look surprisingly different from one day to the next. Lighting, camera angle, facial expression, makeup, and even the time of day can dramatically change how we appear in photographs.

But every so often, a result comes along that genuinely makes you stop scrolling.

The 👀 are often the first feature people notice, and heavy upper eyelids can create a tired or hooded appearance. 😴

By carefully restoring a more open, refreshed eyelid contour, upper lid blepharoplasty can have a remarkable effect while still looking natural and harmonious.

So yes, photographs can play tricks on us.
But sometimes the transformation really is that impressive.

And when the patient is delighted with the outcome, that's what matters most. ✨

Skin - One of the most complex structures in the universe!

Many people notice a new lump, bump, wart, scaly patch, or pigmented lesion - appearances can be deceptive, especially in areas that may be prone to sun damage.

Common skin lesions include:

• Actinic keratoses – rough, sun-damaged patches that can occasionally progress to skin cancer
• Seborrhoeic keratoses – benign age-related growths that can resemble more serious lesions
• Basal cell carcinomas (BCCs) – the most common form of skin cancer, often appearing as a persistent sore, shiny nodule, or non-healing lesion
• Squamous cell carcinomas (SCCs) – potentially more aggressive skin cancers that require prompt diagnosis and treatment

Increasingly, we see lesions being treated with creams, freezing treatments (cryotherapy), or other destructive methods without a definitive diagnosis first being established.

While these treatments have an important role in carefully selected cases, destroying a lesion without obtaining tissue for analysis can mean an underlying skin cancer is missed or its diagnosis delayed.

At Refine Surgical Treatment Centre, patients are assessed by experienced doctors, including NHS Consultant Surgeons on the GMC Specialist Register, who are trained to recognise suspicious lesions and determine the most appropriate management.

Where indicated, we can perform:

✓ Diagnostic biopsies
✓ Complete surgical excision of lesions
✓ Histological analysis of tissue specimens
✓ Definitive treatment of benign and malignant skin lesions

Histology remains the gold standard for diagnosis. It allows the tissue to be examined under a microscope, confirming exactly what the lesion is and ensuring that more serious conditions are not overlooked.

With increasing lifetime sun exposure and an ageing population, skin lesions are becoming ever more common. Whether it appears to be a wart, mole, cyst, skin tag, actinic keratosis, or another lesion entirely, obtaining an accurate diagnosis is often the most important step.

When it comes to skin lesions, the question should not simply be "How do we remove it?" but "What exactly is it?"

What is Lagophthalmos? 🤔

One of the most challenging aspects of upper lid blepharoplasty is determining exactly how much skin, orbicularis muscle, and fat should be removed while accounting for the temporary changes that occur immediately after surgery

In the early postoperative period, it is not uncommon to observe a small degree of lagophthalmos (incomplete eyelid closure).

This can result from a combination of tissue swelling, local anaesthetic 💉 effects, and temporary weakening of the orbicularis oculi muscle. These factors alter eyelid function in ways that must be anticipated before a single incision is made.

The true skill in blepharoplasty lies not in removing tissue, but in understanding how the eyelid will function once swelling resolves and normal muscle activity returns.

If too little tissue is removed, the result may be inadequate correction of hooding and excess skin.

If too much is removed, the consequences can be far more significant, leading to prolonged lagophthalmos, exposure-related discomfort, dry eye symptoms, corneal damage, and, in severe cases, sight-threatening complications.

What makes this particularly important is that some degree of orbicularis weakness caused by local anaesthetic infiltration and surgical manipulation may persist for up to 24 hours after the procedure.

During this period, careful postoperative management, ocular lubrication, and appropriate monitoring are essential to protect the cornea while normal eyelid function returns.

Upper eyelid blepharoplasty is often perceived as a relatively straightforward cosmetic procedure.

In reality, it is a delicate functional operation measured in millimetres 📏 , where surgical judgement, anatomical understanding, and meticulous postoperative care are critical to achieving both a beautiful and safe outcome.

The ⭐ best blepharoplasty results are achieved when aesthetic enhancement never comes at the expense of eyelid function or ocular health.

Precision is everything in blepharoplasty. 🌟

Before surgery begins, meticulous measurements are taken to determine exactly how much eyelid tissue can be safely removed.

The goal is to create just the right amount of tension — enough to cause a slight brow movement and allow the lashes to evert naturally, but never so much that it risks lagophthalmos (the inability to fully close the eyelids).

In this case, we are removing over 50% of the eyelid tissue 😮 to achieve the best possible functional and aesthetic result.

This level of correction requires exceptional surgical judgment, skill, and experience. Blepharoplasty is far more than simply removing excess skin — it is a highly precise balance of science, anatomy, and artistry. 🖊️
