Yesterday we had a visit from two Student Nurses from Dr Gray's, who are heading up a new pilot scheme for patient discharge. They hope to create a useful tool to help reduce the worries of leaving hospital, and to empower patients and their families in what happens next.

They left with lots of positive signposting, examples of Carer concerns and a more rounded understanding of services available for carers in the area.
