Meet our dedicated care home team
💚 Behind great care is a great team.
As our Enhanced Health in Care Homes Service continues to grow, we wanted to introduce the people behind it.
Our dedicated team brings together GP, Advanced Nurse Practitioner, nursing and care coordination expertise, working closely with care home staff to provide proactive, personalised and joined-up care for residents.
A big welcome to our recently recruited GP Dr Bibi, who joins Kathy, Sasha, Susie, Sacha and Kim in supporting our care home residents.
We’re incredibly proud of the team we have built and the relationships they continue to develop with residents, families, care homes and our wider community services. 💙

Welcoming Willowmere Lodge
💙💚 Some exciting news from Hope Farm Medical Centre…
From September, we’re delighted to be commissioned by One Ellesmere Port Primary Care Network to provide the Enhanced Health in Care Homes Service at Willowmere Lodge.
Willowmere Lodge becomes the sixth care home supported by our dedicated care home team.
For residents, this means an enhanced level of proactive, coordinated care, with our team working closely with the care home and wider health and care services to support their individual needs.
We’re really looking forward to welcoming the residents and staff of Willowmere Lodge and building another strong care home partnership.

At Hope Farm Medical Centre, listening to our patients is at the heart of how we improve our services.
Our new Patient Experience Report 2026 brings together what you have told us, what we're doing well, and the changes we've made in response to your feedback.
We're delighted that the National GP Patient Survey showed strong results, including:
💙 94% said our Patient Services Team were helpful
💚 94% reported confidence and trust
💙 97% felt involved in decisions about their care
💚 88% rated their overall experience as good

💙 HPV self-testing – an alternative check for a virus called human papillomavirus (HPV)
Cervical screening remains one of the best ways to protect yourself from cervical cancer. However, if you’ve previously been invited but haven’t attended, the NHS may now offer you an HPV self-test as an alternative.
Invitation to take part in HPV self-testing is initially limited to an eligible group of women aged 30 to 65, who have not attended cervical screening following previous invitations.
The free test can be done yourself, in private at home, using a simple swab which you then return to the NHS by post.
📱 If you’re eligible, the NHS will contact you through the NHS App, text, email or letter, with information on how to order your free kit.
If high-risk HPV is found, this does not mean you have cervical cancer. You’ll be invited to book a cervical screening appointment so that further checks can be carried out.
⭐ Regular cervical screening remains the best option when you’re invited. But if you haven’t attended, HPV self-testing is the next best option.
And remember: you can still book your cervical screening appointment at any time – even if your invitation was months or years ago.

We’re pleased to confirm that our FLU and COVID vaccination clinics are going ahead as planned, with a range of clinics throughout the week and at weekends.

While there are reports of vaccine shortages, our GP Practices across the Ellesmere Port neighbourhood are working collaboratively together, making our local services more resilient and helping us maintain access to vaccinations for our community.

By working together as a neighbourhood, we can offer more flexibility and make it easier for people to get protected.

5. close contacts of immuno-compromised individuals
6. front line care workers who don’t have an occupational health scheme

⭐️You are eligible for a COVID VACCINE if you are:

1. aged 75 years and over, or 74 but turning 75 by the 30th June
2. aged 18 years to 74 years and immunosuppressed
