πŸ“’ FreeStyle Libre 2+ Sensor Users

We've recently had a number of patients tell us that their FreeStyle Libre 2+ sensors have either fallen off early or experienced technical issues. While the recent hot weather may have contributed in some cases, these problems have been reported more widely over the past few months.

If your sensor is faulty or comes off before it should, please request a replacement directly from Abbott, as they have a dedicated replacement service for eligible sensors. This is the quickest way to receive a replacement and helps reduce delays.

You can request a replacement here:

Get help if your FreeStyle Libre sensor fell off, stopped working, showed an error message, or experienced another issue. Start your support or replacement request online in just a few steps.

πŸ“’ Our 2026 GP Patient Survey Results
Firstly, we would like to say a huge thank you to everyone who took the time to complete the NHS GP Patient Survey. Your feedback is invaluable and helps us understand what we are doing well and where we need to improve.

This year, 89% of patients rated their overall experience of Oaklands Health Centre as good. Whilst we are pleased that this remains well above the West Yorkshire ICS average (76%) and the national average (77%), we are naturally disappointed that our overall satisfaction score has fallen from 95% in 2025 to 89% in 2026.

The last 12 months have been a period of significant change for the practice. We have welcomed a new GP partnership, introduced a new clinical computer system (SystmOne), implemented a new telephone system and continued to develop our services to meet the needs of our growing patient population. We recognise that periods of change can sometimes affect patients' experiences and we would like to thank everyone for their patience and understanding while our team adapted to these new ways of working.

Despite these changes, we are incredibly proud of many of this year's results:

⭐ 94% of patients found our reception and administrative team helpful.
⭐ 97% had confidence and trust in the healthcare professional they saw or spoke to.
⭐ 98% felt involved in decisions about their care and treatment.
⭐ 93% said their healthcare professional listened to them.
⭐ 92% said they were treated with care and concern.
⭐ 84% found it easy to get through to us by phone – significantly higher than both the ICS and national averages.
These results are a testament to the commitment, compassion and resilience of our entire team, who have worked tirelessly throughout a year of considerable change.

Although we are disappointed that our overall satisfaction score has fallen, we are absolutely committed to improving it. We will continue to listen to your feedback, review our services and make further improvements to our appointment systems, access and patient experience. Our ambition is to regain and exceed last year's excellent results.

Thank you for your continued support and for placing your trust in Oaklands Health Centre. Together, we will continue striving to provide the highest quality care for our community. πŸ’™

NHS App Update

We are aware that some patients are experiencing issues with the NHS App following the installation of our new clinical system.

Our IT teams are working hard to resolve these issues as quickly as possible and we appreciate your patience while this is being investigated.

If you are experiencing problems accessing the NHS App, please try the following:

- Log out of the NHS App.
- Remove (delete) the app from your device.
- Download and install the NHS App again.
- Log back in using your NHS login details.

For many patients, this has resolved the issue.

If you continue to experience problems after trying these steps, please let us know by contacting the practice.

Thank you for your patience and understanding while we work to get everything running smoothly.
