You could see it before we even touched him neck locked up, barely any rotation.

That restriction isn’t random. It’s usually a joint that’s lost some of its normal glide, with the surrounding muscles guarding around it. The “crack” you hear afterwards is a cavitation, gas releasing from the joint fluid as pressure changes. It’s not damage, and it’s not really “the treatment” either it’s a byproduct of restoring proper movement to a joint that was stuck.

We find exactly where the restriction is, treat it, then re-test to confirm range has actually changed. Not just guess and hope.

That “crack” isn’t bones out of place — it’s a facet joint cavitation, a quick pressure change that releases gas trapped in the joint fluid.

But the real magic happens after: the sudden stretch fires off mechanoreceptors around the joint, which sends a signal to the brain that reflexively relaxes the surrounding muscle and dials down pain perception (a bit like resetting the nervous system’s “tension” setting).

That’s why patients often feel looser and calmer straight after — it’s not just the sound, it’s the neurophysiology.

His deadlift numbers don’t lie, but they don’t tell the whole story either.

This RDL iso hold is part of his rehab progression, and that shake isn’t just fatigue. It’s his hip losing control in two directions at once — anti-flexion and anti-adduction — meaning the hip can’t resist collapsing forward and inward under sustained load. Under a 1RM, he can brace and compensate his way through it. Hold it still with no momentum to hide behind, and it shows up instantly.

5 months post-op patella tendon repair and this is elite-level progress. 💪

Today’s session: lateral step downs, pilquo step downs, and quad extensions loaded at 15kg. Why this matters:
→ Lateral & poliquin step downs build single-leg control and eccentric strength through the exact range that patella tendons need to tolerate under real-world load — deceleration, stairs, cutting.
→ 15kg quad extension at 5 months is a serious strength marker. Most post-op knees aren’t anywhere near this load this early — it shows the tendon is remodelling well and the quad is firing properly again.

This is exactly the kind of data-led, criteria-based progression we build every rehab plan around — assess, load, re-test, repeat. Incredible work from this patient, the consistency is showing. 🔥
