Many people assume under-eye bags are simply a sign of tiredness, but the reality is often more complex.

As we age, the tissues supporting the lower eyelid can weaken. The fat that normally cushions the eye may gradually move forward, creating the appearance of puffiness or “bags” beneath the eyes.

Other factors can make them more noticeable, including:

• Ageing changes in the eyelid tissues
• Fluid retention
• Allergies
• Lack of sleep
• Changes in skin quality and elasticity

Why can two people of the same age look completely different?

Most people assume the answer is genetics.

Genetics certainly play a role, but they’re only part of the story.

Facial ageing is driven by a combination of factors, including sun exposure, smoking, weight fluctuations, stress, sleep quality and general health. Over time, these influences affect the skin, fat, muscle and even the underlying bone structure.

Importantly, not everybody ages in the same way.

Some people predominantly lose facial volume. Others develop skin laxity. For some, skin quality and sun damage become the most noticeable changes. Most people experience a combination of all three, but in different proportions.

This is why two people can be exactly the same age yet look remarkably different.

It’s also why there is no universal solution to facial ageing. Understanding how a face has changed is usually far more important than knowing how old someone is.

What’s the difference between skin ageing and volume loss?

Many people use the word ageing to describe everything they see in the mirror, but ageing is actually a combination of different processes.

Skin ageing refers to changes in the skin itself:
• Fine lines and wrinkles
• Changes in texture and elasticity
• Thinning of the skin
• Sun damage and pigmentation

Volume loss happens beneath the skin:
• Loss of facial fat and soft tissue support
• Flattening of the cheeks
• Hollowing around the eyes and temples
• Less support for the skin, leading to folds and jowls

This is why two people of the same age can look very different. One may have excellent skin quality but significant volume loss, while another may have minimal volume loss but more skin wrinkling.

Understanding which changes are driving your appearance is important because different concerns often require different approaches. Improving skin quality won’t restore lost facial support, and replacing volume won’t necessarily address skin texture or sun damage.

Most facial ageing is a combination of both.

One of the most common concerns I hear during consultations is:

“Will everyone see the scar?”

The reality is that scars are often far less noticeable than patients expect.

That’s because scar appearance depends on much more than the incision itself. Careful planning, strategic placement, meticulous closure and the body’s natural healing process all play a role.

Most scars are at their most visible in the early weeks. Over the following months they typically soften, flatten and fade, often blending surprisingly well into their surroundings.

No scar is ever completely invisible, and every patient heals differently. However, with thoughtful surgical planning and time, scars are rarely the feature that draws attention.

It’s a statement that often surprises patients.

One of the biggest misconceptions in facial rejuvenation is that fillers and facelifts achieve the same thing. In reality, they address different aspects of ageing.

Fillers are designed to restore volume and can be an excellent option for carefully selected patients. However, as we age, it’s not just volume that changes. Tissues descend, the jawline softens, jowls develop, and skin laxity becomes more apparent.

In these situations, adding more volume may not address the underlying cause and can sometimes make the face appear heavier rather than more refreshed.

Sometimes they’re right.

But the neck doesn’t age independently from the rest of the face.

Changes in the cheeks, jawline and deeper facial tissues can all influence what we see lower down. If those changes aren’t considered, treating the neck alone may not address the underlying cause of the concern.

This is why consultation is so important. The goal isn’t simply to treat the area that bothers a patient most, but to understand what is contributing to that appearance in the first place.

For some patients, a neck lift alone may be entirely appropriate. For others, a different approach may be recommended.

A facelift doesn’t stop ageing. It simply turns the clock back.

One of the most common misconceptions about facelift surgery is that it somehow “stops” the ageing process. It doesn’t.

A facelift repositions and supports tissues that have descended over time, restoring definition and addressing some of the changes associated with ageing. After surgery, however, your face will continue to age naturally.

The aim isn’t to look like a different person. It’s to look like yourself, just refreshed.

How long results last varies from person to person and depends on factors such as skin quality, genetics, lifestyle, sun exposure and ongoing changes that occur with time.

The best facelift results are often the ones nobody can quite put their finger on. You simply look well, rested and more like the version of yourself you feel inside.
