Mastopexy - full lift and reshaping:
This patient had very impressively lost 20 kg through diet and exercise and completely transformed her body. But despite all that hard work, she was left really unhappy with what had happened to her breasts.

The change over the years, and then further after weight loss was significant. There was considerable skin laxity, loss of upper pole support, and the breast tissue had descended and spread laterally. She described feeling that when she lay down, the breasts would simply fall out towards the sides of her chest.

What she wanted was simple: for her breasts to match the body she had worked so hard to achieve!

On examination, this wasn’t just a case of ni**le position. The entire breast envelope had stretched, and the tissue had descended on the chest wall. This required a proper reshaping of the entire breast, not just skin removal.

I performed a deep plane breast uplift using her own tissue, without implants. The aim was to rebuild internal support, re-drape the skin, and restore a more natural breast shape and position on the chest.

At 16 weeks post-op, the early result is already amazing. The breast is shorter, more supported, and sits properly on the front of her chest with improved projection. Importantly, this has all been achieved using her own tissue.

For me, a good breast uplift is about moving the ni**le plus restoring structure, support, and proportions, so that the breasts feel as good as they look.

These scars are still early and will continue to soften and fade over time. The vertical ones are already very feint!

Her words afterwards were simple: “amazing”, “life-changing”, “like magic”.

And perhaps most importantly, she told me she only wishes she had done it years ago.

After everything she had achieved with her weight loss, she finally feels her breasts now match the rest of her body.

A substantial breast reduction, with just over 2.1 kg removed.

This lady had lived with very large, heavy breasts for most of her adult life. They caused significant neck and back discomfort, difficulty exercising and finding clothes that fitted properly, and considerable embarrassment and self-consciousness.

Her starting point was a very large breast volume with severe ptosis, extensive skin excess and significant descent of the breast tissue.

In a reduction of this scale, simply removing breast tissue is not enough. A large amount of excess skin also has to be removed and the remaining breast tissue carefully reshaped and repositioned to produce a smaller, lifted and properly supported breast.

I removed 1.1 kg from the right breast and 1.0 kg from the left, a total reduction of just over 2.1 kg.

There has been a substantial reduction in both breast volume and weight, but equally important is the change in shape. The breast footprint is considerably shorter, the lower pole has been reduced and tightened, and the remaining breast tissue has been reshaped into a much more elevated and proportionate position on the chest.

At 10 weeks the scars are still relatively early and will continue to mature and fade over the coming months. The breast shape is already excellent and she is able to exercise freely.

For patients with breasts of this size, reduction mammoplasty is an enormously important operation. It combines significant tissue removal with quite extensive breast reconstruction and reshaping, with the aim of improving both physical symptoms and body confidence.

What can a breast uplift achieve without implants?

This is a lovely example of something I often discuss with patients who want their breasts lifted and reshaped, but really don’t want implants.

You can see that this lady had quite significant breast sag, with stretched skin and much of her breast tissue sitting well below the natural breast fold. Importantly though, she still had plenty of her own breast tissue to work with. So she didn’t need an implant to give her projection.

I performed a deep plane breast uplift, extensively reshaping and repositioning her existing breast tissue higher on the chest, whilst also removing the excess skin.

The photographs show her result at just 10 weeks after surgery.

There are no implants and no artificially added volume. The fullness and projection you can see have been created entirely using her own breast tissue.

For me, this is the important part of modern breast uplift surgery. It isn’t simply about lifting the ni**le and tightening the skin. The internal breast tissue itself needs to be properly reshaped if you want to create a shorter, better supported and more youthful breast shape.

And at only 10 weeks, this is still an early result. The breasts will continue to soften and settle, and the scars will continue to fade considerably over the coming months.

Another example of what can be achieved with breast uplift surgery alone.

One of the common misconceptions I hear is that an implant is needed to create fullness and projection when breasts have become significantly droopy. It isn’t always, and can hinder the uplift quality.

This lady had quite marked breast sag, with stretched skin, a long lower pole and much of her breast tissue sitting below the natural breast fold. But she also had a good volume of her own breast tissue to work with.

For me, the important part of an uplift like this isn’t simply removing excess skin and lifting the ni**le, it’s what happens to the breast tissue underneath.

I performed a deep plane breast uplift, extensively mobilising and reshaping her own breast tissue and bringing it upwards into a shorter, better supported breast shape.

There are no implants here and no artificially added volume!

The projection and fullness you can see have been created entirely from her own breast tissue.

These photographs are at just 10 weeks following surgery, so the breasts and scars will continue to settle and mature considerably over the coming months.

I think this is a particularly nice example of why breast uplift surgery is really a reshaping operation, rather than simply a skin-tightening procedure.

The aim is of course to make the breasts sit higher. But it is also to create a breast shape that looks balanced, proportionate and natural for the individual patient!

Many women assume they need implants when their breasts lose shape. Quite often they don’t but you do need to evaluate each patient as an individual!

Pregnancy, breastfeeding and later the menopause all change the breasts dramatically. Over time breasts lose volume in the upper part, the skin stretches and the ni**les gradually slide lower. It happens so gradually that many women simply accept it as part of getting older. And that’s fine. But it really bothers some women.

This lovely lady came to see me because she no longer recognised the shape of her breasts. What she wanted wasn’t to be larger; she said it was to look like herself again.

After examining her, I felt she had enough of her own breast tissue to achieve exactly that without implants. A full deep plane breast uplift allowed me to reshape and support her own tissue, restoring a natural, youthful breast shape.

These photographs were taken just six weeks after surgery, so she still has some tightness to settle and her scars will continue to mature over the coming months.

One of the most important parts of cosmetic breast surgery is deciding what not to do. In this case, adding implants would have added unnecessary weight. The better operation was to restore what she already had.

“I don’t want bigger breasts. I just want them lifted.”

I hear that almost every week.

This lovely lady came to see me in her fifties after years of gradual change following breastfeeding and then the menopause. Her breasts had become lower, flatter and had lost the shape she remembered. It had slowly knocked her confidence.

The important thing was understanding why that had happened and choosing the right operation to correct it.

She didn’t need implants. She had enough of her own breast tissue to create an attractive, natural breast shape. Adding implants would simply have added weight to tissues that had already stretched over time.

Instead, I performed a full deep plane mastopexy, lifting and reshaping her own breast tissue while rebuilding the internal support. Although the scars are what patients see, much of the operation is actually about what happens underneath. That’s what gives the breast its shape and support.

These photographs were taken just six weeks after surgery. She still has some swelling and her scars will continue to fade over the coming months, but the change in breast position, projection and overall shape is already clear.

One of the most satisfying parts of my job is helping women realise they don’t always need implants to achieve the result they’re hoping for. Sometimes, careful assessment and choosing the right operation is the most important part of the entire process.

“I just don’t feel like they’re me anymore.”

Those were the words this patient used when she came to see me, 18 years after her original breast augmentation (done elsewhere)

Her implants hadn’t increased in size, but you can see her breasts had changed considerably over the years. The skin had stretched, the breast tissue had descended and what had once felt balanced and feminine now felt too large, heavy, elongated and out of proportion. She had reached the point where she avoided certain clothes, didn’t like seeing herself undressed and simply wanted to feel comfortable and confident again.

This is something I see quite often and many women think the answer is simply to replace their implants, but revision breast surgery is rarely that straightforward. The real question is why the breasts have changed, because that determines the best solution.

In this case, I removed her original 410 cc anatomical implants and replaced them with smaller 265 cc moderate profile round implants. More importantly, I completely reshaped the breasts using my vertical bipedicle mastopexy technique, restoring support, improving breast position and recreating a natural, balanced shape. Safety is also important here to reduce surgical risk. I have refined this technique over a 20 year period as a consultant and worked with this specific approach for a decade. I regard it as my signature operation now and see patients from all over the UK who want this approach or the permanent removal of the implants with this approach modified further still!

For me, the greatest satisfaction isn’t creating breasts that look “operated on”. It’s helping women feel like themselves again. That almost always comes from careful planning, respecting the tissues and tailoring every operation to the individual, rather than applying the same solution to everyone.

I’m delighted with her progress, and even more pleased that she now feels comfortable, confident and back in proportion again.

years after breast augmentation, this patient no longer recognised her breasts.

Her implants had not become larger, but over time her breasts had changed around them. The skin had stretched, the breast tissue had descended a lot and the overall shape had become longer, droopy and less supported. Although the implants were still 410 cc anatomical implants, her breasts now felt too large, heavy and out of proportion. She wanted them lifted, smaller and natural again.

A common misconception is that some women assume that replacing old implants alone will restore a youthful shape. In reality, that is often only one part of the solution.

In this case, I removed her 18-year-old implants and replaced them with 265 cc moderate profile round implants. More importantly, I completely reshaped the breast using my preferred vertical bipedicle mastopexy technique, allowing me to restore support, improve projection and recreate a more youthful breast shape. This is the major part of the revision and takes around 2.5 hours.

Revision breast surgery is rarely straightforward. Every previous operation leaves a different starting point, and successful outcomes depend on understanding how the breast has changed over time before deciding how best to rebuild it.

For me, the most satisfying part of cases like this is not simply changing an implant, but using my skills to restore balance, proportion and confidence in a way that looks natural and is tailored to the individual patient.

Breast reduction is about much more than making the breasts smaller.

In patients like this, the real challenge is restoring shape after years of stretching. The breast tissue had become thinned, there was a very large excess of skin, and the ni**les had descended significantly.

This operation involved removing approximately 1.1 kg of breast tissue, together with a substantial amount of excess skin. The ni**les were repositioned by around 19 cm, and the areolae reduced to restore a more natural proportion.

A successful breast reduction is not judged simply by how much tissue is removed. It is about carefully preserving and reshaping the remaining breast tissue to create a breast that sits naturally on the chest, whilst maintaining femininity and proportion.

These photographs were taken just 12 weeks after surgery, so the scars are still relatively early and will continue to mature over the coming months.

Every breast reduction requires an individual surgical plan. The amount of tissue removed is only one part of achieving a balanced, long-lasting result.

A successful explant is about far more than removing implants.

This lady first underwent breast augmentation elsewhere almost 30 years ago using Trilucent implants. Following their withdrawal from the market, they were exchanged approximately two years later. By the time she came to see me, those replacement implants were themselves around 27 years old.

Over the years her breasts had become progressively larger, heavier and increasingly ptotic. She had also developed pain and distortion of the left breast. At surgery, I found the left implant had ruptured.

Simply removing the implants would have left significant loose skin and a poor breast shape. After careful assessment and planning, I performed bilateral explantation, capsulectomies and dermocutaneous mastopexies to reshape the breasts using her own tissue, without inserting further implants.

These photographs were taken just three months after surgery.

Cases such as this highlight why explant surgery requires careful judgement. Every patient presents with different anatomy, tissue quality and expectations, and the operation should be tailored accordingly. The objective is not simply to remove implants, but to leave the patient with breasts that are balanced, natural in appearance and comfortable.

I’m delighted that she achieved exactly what she wanted: smaller, lighter breasts with an improved shape, without the need for replacement implants.
