The five people in every clinic waiting room are always there, and yes, you are one of them. So am I. There is The Early One, who has basically moved in and has been seated since Tuesday. There is The Researcher, arriving with a laminated list of questions that I genuinely respect, and I will happily answer every single one. There is the Nervous First Timer, texting her sister in capitals while pretending she is fine. Then there is The Regular in loungewear, who knows the biscuit drawer personally and honestly, same. And finally there is The Winger, who books “something, whatever you think” and hands me her whole face, which is flattering and absolutely terrifying in equal measure. Here is what the law does not protect you from, and it is the regulation gap nobody mentions in the adverts. In much of the UK, non surgical cosmetic treatments are far less regulated than most people assume, and training standards can vary wildly from one provider to the next. It means you cannot rely on a badge, a logo, or a pretty Instagram grid as proof that someone is properly qualified. Until the rules catch up, your own checks are the regulation. Ask about medical background, insurance, prescriber access, and what their complications plan actually looks like if something goes sideways. They are boring questions, but they are brilliant protection, and any decent practitioner will be glad you asked. I have these conversations with clients every week, and the ones who ask the awkward stuff are always the ones I feel most confident treating. I hear “can I just have a little top up” all the time, and I get why it sounds simple.
But there really is no such thing as a tiny, casual appointment in my clinic.
Every top up still means a full consultation, a proper assessment, and me having a good look to make sure you have not been overfilled somewhere else.
That “just a little” does an awful lot of heavy lifting in one sentence, and it can hide bigger questions about balance, proportion, and what your face actually needs today.
Sometimes the best plan is a small tweak. Sometimes it is a pause, a dissolve, or simply leaving things alone for a bit.
My regulars know I am fussy about this, and honestly, it is why their results stay soft and believable.

Your consultation should feel like a chat, not a checkout. If someone reaches for a needle in the first ten minutes, I would genuinely want you to walk straight back out. A proper consult with me means we go through your medical history, what is actually bothering you, what you want to spend, and what is realistic. Sometimes the right answer is a treatment, and sometimes it is sleep, SPF, and a moisturiser you will actually use. I have sent people home with nothing but advice more times than I can count, and it is mainly because treating the wrong thing helps nobody and it always shows. I would rather you leave feeling clear and looked after than rushed into something you are unsure about. My honest Monday morning as an injector. Coffee first, opinions second. By six am I am already mentally rehearsing every consultation, long before I have even had my tea. By nine I have told three people their lips are already lovely, and I have also had to explain to one person that no, we cannot make them twice the size by Friday. I love what I do, even the slightly awkward bits, because the aim is always to keep you looking like you, just a bit fresher. If my treatment bed could talk, we would both be in trouble. Fortunately it is bound by confidentiality, same as me. It has heard engagement announcements before the family knew, resignation speeches rehearsed twice, and at least four confessions about who really scratched the car. There is something about lying down under a warm blanket that makes people tell you everything. Half my job is clinical, the other half is being a very discreet human diary with good lighting. I genuinely love that my clinic can feel like a little pause button where you can breathe out and be looked after. So go on then, what would you confess from the comfort of a treatment bed? Fine lines are rarely just about the number on your birth certificate. They are usually about how well your skin is supported day to day. Collagen gives structure, hydration gives bounce, and when either one is running low, lines tend to show up sooner and stick around longer. I see it all the time, clients who swear they are suddenly ageing overnight, when really their barrier is tired and their skin is dehydrated. A proper skin plan strengthens the skin so it can hold on to moisture and stay resilient, rather than chasing quick surface smoothing. That can mean the right home routine, a bit of in clinic support, and patience while your skin remembers how to behave. A few unhinged rules I have as a medical professional. Non negotiable, and yes, people still try to email me about them. I will not treat you on your wedding morning, even if you swear you are calm and it is only a tiny tweak. If you tell me “just do whatever you think”, I will sit there thinking for a very long time, because your face deserves an actual plan, not vibes. If you arrive straight from a row with your ex, we are having tea first and the consultation second. Your nervous system gets seen before your skin does, and I am completely serious about that. It keeps my work safe, predictable, and honestly, it usually gets you a better result anyway. Things you should never be embarrassed about, and yes, you can read this twice. Asking what a treatment costs, crying in a consultation, not knowing what anything is called, wanting to look nice for no occasion whatsoever, being 52 and still not knowing your face shape. I hear apologies for questions, budgets, chin hair, and taking up space, and I promise you do not need to preface your own face with sorry. I have also watched a grown man faint at a blood test, so honestly, perspective helps. My job is to explain things in plain English, go at your pace, and make sure you feel safe while we figure out what you actually want. You are allowed to want things without a permission slip.
