Constipation is not always obvious in an older person.

They may become restless, confused, anxious, lose their appetite or simply seem unlike themselves. Even loose stools can sometimes occur when liquid stool passes around an impaction.

Keeping a simple bowel record can help families notice changes early. If you are concerned, speak to a pharmacist, GP or NHS 111.

Seek urgent medical help for severe or worsening abdominal pain, vomiting, bl00d in the stool or an inability to pass stool or wind.

And please, never attempt to remove impacted stool yourself. This must be handled by an appropriately trained healthcare professional.

Sometimes, the smallest detail provides the biggest clue.

Today’s visit left me thinking about how easily we can overlook the people who appear to be coping.

Independence and needing support can exist at the same time. A person may manage most things well but still benefit from a familiar face, a little reassurance and someone who notices when the day is not going to plan.

Sometimes, the greatest value of a visit is simply knowing that someone was there.

During one of our recent visits, the person we were supporting decided to prepare his own meal. We stayed nearby, kept him company and allowed him to take the lead.

It can be tempting to step in and do everything, especially when it would be quicker. But support does not always mean taking over.

When it is safe, giving someone the time, encouragement and reassurance to continue with familiar tasks can help preserve their confidence, routine and sense of purpose.

Sometimes, the most supportive thing we can do is simply stay close and let them keep doing what they can for themselves.

Many families avoid talking about end of life because they worry it will take away hope.

But planning ahead is not about expecting the worst. It is about making sure someone’s voice is still heard if they later become too unwell to express what they want.

These conversations do not need to happen all at once. Begin gently, choose a calm moment and allow the person to lead.

You might simply ask:

“What would be most important to you if your health changed?”

Then listen without correcting, persuading or rushing them.

It may be an uncomfortable conversation today, but it could bring clarity and reassurance during a difficult moment later.

We can easily overlook these changes because, in our minds, they are still the strong and capable parents we have always known.
