🌟 WOW – What an Incredible Opening Day for SWAGGA’s Neighbourhood Health Hub! 🌟
Monday 1st June marked the official launch of the SWAGGA Neighbourhood Health Hub… and what a day it was! 🙌
We were absolutely blown away by the level of engagement from both local service providers wanting to collaborate and patients eager to access support… and the final footfall says it all:
🔥 Our closing count on day one was close to 550! 🔥
An amazing start and a clear sign of just how needed this service is in our community 💙
💥 Real Impact – In Just ONE Day
Here are just two powerful examples of how the Hub is already changing lives:
🧠 Case Study 1 – Joined-Up Support in Action
A gentleman attended the Hub feeling extremely anxious and struggling with his mental health due to ongoing bullying from neighbours. He felt unheard despite previous reports.
✅ The Housing Support Officer he had previously contacted was at the Hub – leading to an appointment booked for the same day at his home
✅ Offered a Mental Health Nurse appointment
✅ Booked in for Noah’s Ark Men’s Hub the following day
✅ Signposted to a men’s support group at Long Lane Church that same evening
Due to a severe stammer, communication had been a barrier for him – but the Hub removed that barrier by connecting him directly with the right people.
➡️ He is now also booked in with a Social Prescriber later this week to focus on diet and exercise once his housing situation is resolved.
💼 Case Study 2 – Immediate Access to Life-Changing Support
Another patient walked in and accessed multiple services on the spot – and was over the moon! 🎉
Support needed:
• Financial support
• Food bank voucher
• Employment support
✅ CAB advice referral + signposted to local drop-in service (previously unaware of it despite living locally)
✅ Same-day 11am appointment with REACH for employment support
✅ Food bank voucher issued immediately by SWAGGA Social Prescriber
✅ Follow-up arranged to ensure ongoing support
💬 A clear demonstration of how vital, proactive, and responsive the Hub already is.
🤝 Collaboration is Key
We were also pleased to welcome local councillors and secondary care managers, continuing important conversations about building stronger, more connected services for our community.
🚀 What’s Next?
With such a successful launch, we’re now moving into the next phase:
👉 Developing a full timetable of support with local service providers
We already have themed days taking shape (see flyer), including:
✅ Health & lifestyle support
✅ Family help & wellbeing
✅ Learning disabilities, neurodiversity & carers
✅ Ageing well & tackling loneliness
