Deprojection and dorsal hump reduction

It was great to see my patient following her surgery a couple of months ago. Her rhinoplasty result has really transformed her profile, without erasing her individuality. She disliked her relatively prominent nose, as well as her tall bony dorsal hump. She had a longer, slim nasal profile that we were keen to preserve.

Despite involving significant reduction of the nasal profile, the key to operations of this type is reconstruction and tip support. The "architecture" of the nose has to be supported carefully using grafting techniques in order to maintain ideal structural integrity, prevent a loss of tip rotation, and safeguard the nasal airway.

Grateful thanks to my patient for allowing her postoperative outcome to be shared. Hers was a relatively complex primary rhinoplasty with a number of changes to the nasal appearance, but with a transformative and very gratifying outcome.

Her main concern was her profile: she felt that her nose dominated her other features and was too prominent. She wanted her dorsal hump to be removed, but not to create an unnatural "scooped" appearance. On the front view, her nose was symmetrical but she was keen to achieve a less bulbous appearance.

Her procedure involved dorsal hump removal, deprojection of the profile and the tip, nasal bone sculpture and osteotomies, and careful shaping and slimming of the nasal tip cartilages. We were both delighted with how it has turned out!

Subtle profile transformation

It was a pleasure to catch up with my patient in clinic following her rhinoplasty earlier this year. She is a perfect example of how even a small change can transform how we perceive our nose. She had always disliked her dorsal hump, but like many patients, wanted to preserve the other nice features of her nose.

We planned a procedure using photo morphing, focused on careful reduction of the bridge and slight deprojection of the nasal tip, keeping other aspects nice and stable. The final result is flattering, feminine and completely "under the radar"!

Natural masculine rhinoplasty

Many thanks to my patient for sharing his postoperative photos. He underwent septorhinoplasty under my care for both functional and cosmetic concerns. He had a significant septal deviation which was affecting his nasal airway. At the same time, he had never liked his nasal appearance, and in particular the downward-pointing and overprojected profile and tip.

Małe rhinoplasty requires careful judgement in order to balance cosmetic and functional concerns, and to avoid an "operated" look caused by excessive reduction. My patient's tip was stabilised and rotated to a natural 90-degree angle. The profile was conservatively reduced in combination with a small augmentation at the root of the nose (the radix) in order to maintain a strong appearance.

Long-term result: dorsal hump removal and natural tip rotation.

I was very pleased to see my patient in clinic for a longer-term review after 7 months. Her rhinoplasty result has held up beautifully, with a natural and feminine profile and stable rotation of the nasal tip. The improvement in her tip definition has become much more apparent with time.

Long-term result: deprojection and dorsal hump reduction

I had the opportunity to catch up with my patient, who is now four months post-op after her rhinoplasty.

Many patients are concerned that a good early rhinoplasty outcome might deteriorate over time, especially with regard to drooping of the nasal tips. However, modern rhinoplasty technique is focused on natural appearances, good nasal function and long-term stability. The universal use of cartilage grafting techniques helps to stabilise the new bridge of the nose, and to support the nasal tip to prevent de-rotation over time.

In my patient's case, you can see that the tip was deprojected and refined, maintaining a stable, feminine, slightly open tip angle. This is the cornerstone of a natural and sustainable outcome. On the frontal view, despite the removal of her dorsal hump, we can see excellent refinement and slimness of the nasal bridge.

"It looks like the nose I was supposed to have"

Grateful thanks to my patient for allowing her postoperative outcome to be shared. She had multiple concerns regarding the appearance of her nose. She had sustained an injury in childhood which had led to noticeable nasal asymmetry. She also disliked her bulbous nasal tip, which was slightly droopy and descended more on smiling. Her preferred outcome was a less prominent, more feminine profile, with a smaller tip and better symmetry.

Multiple adjustments were made during surgery: dorsal hump reduction, asymmetrical osteotomies, septoplasty and a septal extension graft for tip support, and comprehensive reshaping of her tip cartilages.

It was hugely gratifying to see her for her follow-up appointment after around two months. Her nose looks beautiful - and most importantly blends perfectly with her other features.

Sometimes a rhinoplasty consisting of a number of small adjustments, nevertheless results in an outcome that is much more than the sum of its parts. My patient had a small dorsal hump, a slightly low nasal tip angle, and mild nasal asymmetry. In combination these features led her to request a rhinoplasty for a softer look.

Her result shows a classic feminine profile, without any hallmarks of having had surgery!

Profile and tip perfection

There are certain "ideal" hallmarks that an experienced rhinoplasty surgeon will try to create for patients who want a classic, natural feminine profile. These include a straight nasal bridge with a gentle supra-tip break, a well defined tip, and a slightly open nasolabial angle.

My patient disliked her dorsal hump, which was accentuated by her low nasal tip and hanging columella. Careful structural reduction of the bridge was performed, along with tip support using a septal extension graft, lifting the tip without excessive nostril show.

Transforming the nasal profile and tip

I was delighted to see this wonderful result recently in clinic, which transforms my lovely patient's profile. She disliked her tall dorsal hump and overprojected profile, as well as her bulbous nasal tip.

We used preoperative simulation to help us design the profile that suited her wishes. Her procedure involved substantial reduction of the nasal bridge, along with support and slight rotation of her nasal tip.

You can see the refinement of her nasal tip in the final image. The tip cartilages were slightly narrowed, and their "resting angles" adjusted using suturing techniques, for the perfect amount of definition.
