We are delighted to welcome Consultant Surgeon Hayley to the London Vascular Clinic.

With extensive experience across the full spectrum of arterial and venous treatments within the NHS, Hayley is now available for private consultations at our Chelsea clinic.

Her distinguished academic and clinical background includes serving on the Council of the Royal Society of Medicine’s Venous Forum, the UK’s specialist body for venous disease. She is also Co-Leader of the internationally renowned Charing Cross Venous Training Workshop, one of the world’s largest annual training events in venous medicine.

Hayley brings specialist expertise in:
• Varicose vein treatment
• Deep vein thrombosis (DVT)
• Deep vein obstruction
• Complex venous conditions

She also leads the leg ulcer service in her NHS hospital and now heads our multidisciplinary private leg ulcer service on Sydney Street.

A Conferência Internacional de Flebologia Estética IMAP tem início esta semana em São Paulo, Brasil.

Presto homenagem ao fundador da IMAP, meu amigo Professor Kasuo Miyake, que me ensinou como realizar o CLaCS, método por ele inventado para tratar varizes cosméticas. Aprendi imensamente com Kasuo e sua equipe. Grande parte do que fazemos em Chelsea baseia-se na filosofia e métodos desenvolvidos em sua clínica e aprendidos ao seu lado.

Kasuo merece congratulações por criar a principal conferência mundial de flebologia estética. Foi um prazer participar do congresso da IMAP em anos anteriores, e aguardamos ansiosamente a próxima oportunidade.

The International Meeting for Aesthetic Phlebology IMAP starts this week in Sao Paulo, Brazil.

I pay tribute to the IMAP founder, my friend Professor Kasuo Miyake, who taught me how to do CLaCS, the method he invented to treat cosmetic thread veins.
I learnt so much from Kasuo and his team. Much of what we do in Chelsea is based on the philosophy and methods developed in his clinic and learned at his side.

Kasuo must be congratulated on creating the world’s premier aesthetic phlebology meeting. It has been my pleasure to participate in the IMAP congress in past years and we look forward to the next time.

We wish you all the best for IMAP 2023!

Tremendous start to the Vascular Society and Royal Society of Medicine Venous Forum meeting in Dublin.

First day: working with colleagues teaching the vascular trainees on the VSGBI ASPIRE venous workshop.
Today, moderating the presentations of original venous research, followed by updates on venous controversies and the expanding role of specialist nurse practitioners. More top presentations on aortic, carotid and peripheral arterial disease capped by a thoughtful session on clinical leadership.

Wonderful to see so many friends and mentors again, some known for nearly 30 years. Grateful for the strong support from industry partners too.

This week marks 20 years that Caroline Day Bevan and I have worked together.

From 2003 to 2013 she was central to running the department of vascular surgery at Charing Cross Hospital, and for the last ten years she has been the main practice manager of the London Vascular Clinic.

She is by far the most important professional relationship of my career and is indispensable to the running of our practice. Her skill set and work ethic are stellar. Even more important, all those of us lucky enough to have worked with her over the years treasure her company and her wise and kind personality.

Over 24 million people in the UK have been vaccinated against covid-19 with no discernible increase in blood clots in those receiving a jab.

AstraZeneca have reported report 37 blood clots in 17 million people vaccinated across Europe with the Oxford covid-19 vaccine.

The risk of developing a blood clot is therefore far higher if you are overweight, pregnant, taking oral contraception/HRT, or simply sitting in your seat on a long-haul flight.

Covid-19 is notorious for causing blood clots. The is not.

COVID-19 related thrombosis

COVID-19 has a strong tendency to cause blood clots. The consequences of a clot depend on where it occurs, for example, clots in the heart cause heart attacks

Respiratory symptoms are the main feature of covid-19, but the disease is also associated with a strong tendency to form blood clots. These clots can form anywhere in the body, but the most common sites are the legs and lungs.

The problem is much worse with patients hospitalised with covid-19. Around 20% of covid-19 patients in hospital develop a blood clot, rising to over a third of those in intensive care. Blood clots are associated with a much worse prognosis for covid-19 patients.

Clots in legs (deep vein thromboses) are the commonest type of blood clot seen in covid-19. Many of these start in the community prior to hospitalisation. Prompt treatment with blood thinners helps recovery.

We specialise in the diagnosis and treatment of deep vein thrombosis (DVT). This is a blood clot in the veins of the lower limb. The commonest symptoms are pain in the leg, swelling and discolouration.

DVT can arise spontaneously but some people are particularly prone, for example if there is a family history of blood clot problems. Other risk factors include being overweight and treatment with hormones including the contraceptive pill. Anything which reduces activity such as illness or injury can precipitate a DVT. Covid-19 has a particular propensity to cause blood clots.

DVTs start small but expand and spread causing damage to the circulation. The most worrying kind causes blood clots to lodge in the lungs which can be fatal.
It is important to get a quick diagnosis so that treatment can start before the clot causes harm.

The diagnosis requires examination by a and a doppler ultrasound scan. Treatment is usually with blood thinners.

Important COVID-19 treatment progress

Tremendous news today from UK investigators showing that dexamethasone (an inexpensive widely available steroid) reduces deaths by one third in severely ill ventilated patients and by 20% in non-ventilated patients receiving oxygen. This is the first trial to demonstrate a treatment that reduces mortality from COVID-19. The treatment can be implemented worldwide immediately and will likely save thousands of lives.

The RECOVERY trial is a multicentre study based in Oxford UK with 176 collaborating centres across the country and over 11500 patients recruited so far. It was set up very rapidly at the start of the pandemic in the UK and is designed to assess potential treatments for COVID-19 and compare them with standard supportive therapy. Treatments still under assessment are:

Lopinavir-Ritonavir (commonly used to treat HIV)
Azithromycin (a commonly used antibiotic)
Tocilizumab (an anti-inflammatory agent)
Plasma collected from COVID-19 survivors

Hydroxychloroquine was also studied but that arm of the trial was discontinued last month when it became clear that there was no discernible benefit and more patients died taking hydroxychloroquine than in the control group.
