🎉 Congratulations to the Sutton Health and Care Reablement Unit Team on being shortlisted for the Transforming Care for Older People Award at this year's HSJ Awards! 👏

Since opening in November 2023, the team has been delivering intensive, short-term, therapy-led care to help patients regain independence, improve their functional ability and transition safely from hospital to home.

Through personalised goals, twice-daily therapy sessions, group activities and support with everyday tasks, patients build confidence, maintain independence and reduce ongoing care needs after discharge. 💙

This nomination recognises the team's dedication to helping patients regain independence, improve their recovery, and return home with confidence.

⭐ A huge congratulations to the team on this fantastic achievement, and the very best of luck at the awards!

Come and join our Sutton 0-19 Children's Services and Sutton Best Start Family Hubs for a teddy bears picnic on Monday 10 August at Cheam Park for support and advice with preparing your child for school.

Bring your lunch, your teddy and join us for some fun activities including:

🧸Hoops and bean bags
🧸Story time with Sutton libraries.
