Meet Graham! Eight weeks post deep plane facelift, deep neck lift, lateral brow lift and bilateral upper and lower blepharoplasty.

Having recently turned 60, Graham came to see me wanting to look fresher, healthier and less tired, while remaining masculine, natural and recognisably himself.

To address his concerns around facial and neck laxity, heaviness around the eyes and changes in skin quality, I performed a deep plane facelift and neck lift, alongside a lateral brow lift and upper and lower blepharoplasty.

Thank you to Graham for allowing me to share his experience and these early results.

One of the factors that had previously held him back from surgery was the fear that he might lose some of his masculine character, or simply no longer look like himself.

Male facial rejuvenation requires a particularly measured approach. In Graham’s case, we deliberately kept the changes around the upper and lower eyelids subtle and nuanced. The aim was not to alter the character of his face, but to restore definition and bring his appearance back into harmony with how youthful he feels.

At eight weeks, his neck and jawline are significantly more defined and his eyes appear fresher and less tired. Most importantly, he remains entirely recognisable, with his masculinity and authenticity preserved.

This remains an early result. His scars are still pink and will continue to settle and mature over the coming months.

My facelift work was recently featured in Episode 3 of Olivia Attwood: The Price of Perfection, Facelifts Under 50.

The episode explores the growing conversation around people considering facelift surgery at a younger age.

It was great to see this area of surgery discussed openly in more detail and to have some of my work featured.

Do you think attitudes towards having a facelift before 50 are changing? I would be interested to hear your thoughts.

One of the biggest concerns patients have when considering a facelift is whether they will still look like themselves afterwards.

A natural-looking result should never feel standardised. Each procedure is planned around the patient’s facial anatomy, proportions, pattern of ageing and individual goals.

The aim is not to create a different face, but to restore balance and definition while preserving the features that make each patient recognisably themselves.

If you’re considering facelift surgery and have any questions about achieving a natural-looking result, please leave them below.

Scar placement and quality are considered from the very beginning of every facelift I perform.

Although no surgery can be completely scarless, incisions can be carefully positioned within the natural contours around the ear and hairline, helping them become far less noticeable as they heal. In patients who do not require skin excision, an endoscopic facelift can be performed, where small scars are completely hidden within the hair.

Around the hairline, I perform trichophytic incisions and carefully angle the incision so that hair can grow back through the scar, helping it blend naturally rather than leaving a noticeable line.

Facelift scars will usually look more visible during the early stages of recovery, but they continue to soften and fade over the following weeks and months.

Good aftercare matters too. Moisturising, massage, silicone treatment and daily SPF 50 can all support the healing process and help achieve the best possible result.

The aim is not to pretend that scars do not exist, but to place and manage them carefully so that, once healed, they are as discreet as possible.

I am delighted to feature alongside my esteemed plastic surgery colleagues Mr Mani Ragbir and Mr Mark Soldin in this Daily Mail article on Ozempic and the changes it can have on your body due to rapid weight loss. Undoubtedly this drug with its active ingredient Semaglutide plays an important role in patients with diabetes or obesity. They confer several health benefits including reducing high blood pressure , reducing number of heart attacks and strokes and in some cases reversing types 2 diabetes. So patients prescribed this medication as part of their treatment for diabetes should continue to take advice from their doctor and continue the medication. However there is a trend for off label use for patients who turn to this drug for modest weight loss that could be achieved more gradually with exercise and improved diet. Notable celebrity examples of people who have used this drug include who tweeted recently about it. For this group I think it is important to be informed about the unwanted changes it can have and how you might deal with them. In my area of facial rejuvenation facial volume is a feature of being youthful and so weight loss from the face can lead to a more aged appearance and reveal underlying structural facial laxity which many people don’t like. Being well informed is key !

Thank you Barney Calman and for the open dialogue, let’s continue these important conversations.

Thanks to my beautiful patient, in her early 60s, for allowing me to share her results from lower blepharoplasty with facial fat grafting. One of her main complaints was people thought she looked tired even when she was feeling fresh.

Facelifts address the lower third of the face, jawline and neck; it has little impact on the central face. Rejuvenation of the central face is about lower and upper eyelid surgery and restoring lost volume through ageing - in this case with fat grafting the body’s natural filler. This is why many patients often choose to combine facelift surgery with eyelid + brow surgery and facial fat grafting. In addition to eyelid surgery, my patient had a deep plane facelift combined with facial fat grafting (not shown here.)

This approach of combining deep plane facelift surgery with fat grafting called deep lift and restore or lift and fill facelifts address two of the main underlying mechanisms of facial ageing simultaneously: Structural laxity and loss of facial volume. This approach delivers the most natural and long-lasting results. When combined with lower eyelid surgery, facial fat grafting has a tremendous impact, leading to a comprehensive natural-looking rejuvenation.
As we age, there is a separation between the lower eyelid, cheek and mid-face fat, and loss of facial fat. We often see the tear trough and hollowing in the under eye. Fat contained around the eye can bulge forward and lead to the appearance of lower eyelid “bags”.

To achieve a youthful smooth transition, I redraped fat from the lower eyelid bag over the tear trough to blend the cheek and eyelid, restoring volume to achieve a smooth and natural result. She looks much more youthful and well-rested and is delighted with her results.

Surgeons can perform this surgery through an incision in the skin under the eyelash line called a “subciliary” approach or from the inside of the eyelid called a “transconjunctival” approach which leaves no scars on the skin. When any excess skin needs to be removed, a subciliary approach works well as is the case for my patient here - scars are barely noticeable at 4 months.

I’m feeling proud to be featured in Country & Town House’s May issue. In discussion with we discuss the unrealistic expectations for non-surgical treatments and how they may sometimes lead to an unnatural look. We also explore the difference between deep plane facelifts which I recommend for the majority of patients for the most effective and long lasting results and SMAS facelifts, which I advise occasionally for older patients who may have coarse wrinkles. We also discuss the rise in interest we are now seeing from younger middle-aged patients looking for facial rejuvenation.

The issue is available now to read, let me know if you spot it! Thank you Annabel Jones and for the discussion.

Thank you to my 60 year old patient for allowing me to share his early post operative rhinoplasty result.

Rhinoplasty and most of plastic surgery is not just about technique but judgement and artistry . For some this rhinoplasty result will be too subtle. The nose could have be made smaller, more up turned or more defined but for this particular patient subtlety was key and he is delighted with his early result. There is no cookie cutter approach.

His main concerns were a droopy and bulbous tip, a dorsal hump and difficulty breathing from his right nostril. He wanted to have a natural looking result while retaining his masculinity and importantly to not look too feminised or stylised.

His nose is in proportion to his overall look. The tip is slightly lifted and better defined and will be more so as swelling comes down over the next year . His bony hump has been reduced and the dorsum is straight. His breathing has improved significantly after straightening the nasal septum.

During your rhinoplasty consultation in London I will discuss your expectations and goals for surgery . You will also see digital reconstructions of your nose once photographs are taken to asses your expectations and tailor surgery as much as possible to align with your goals. While an exact outcome based on reconstructions can never be promised 3D reconstructions before surgery are an important way of assessing a patients tastes and expectations. For some a more stylised look is desired and for others subtlety is key. Adjusting for these differences in taste is where surgery and art in cosmetic surgery share the same space.

Changing trends and cultural and gender based differences in taste are an important topic in rhinoplasty. In female rhinoplasty the barbie nose an upturned smaller nose has become popular in some groups whereas for others longer nose with a straight dorsum and a natural aesthetic is sought and seen as elegant and beautiful.
