If your mum or dad suddenly becomes confused overnight, please do not just assume it is “the dementia getting worse”.

A sudden change over a day or two, more confusion, agitation, or simply “not themselves”, is often delirium, not a step down in dementia. And one of the most common, most treatable causes is a urinary tract infection. In older people a UTI often shows up as confusion long before any of the usual symptoms. It gets missed again and again because everyone assumes it is just the dementia.

If you notice a sudden change, here is what to do:

Ask for an infection and UTI check, a urine test and basic observations.
Note exactly when the change started. Sudden really matters, tell whoever assesses them.
Push for a review. Delirium is a medical event, not a mood, and it needs looking at.

Caught early, many people come back to themselves. Left unchecked, it can be mistaken for rapid decline.

Save this so you have it when you need it, and send it to anyone caring for an older parent. It is the kind of thing families tell us they wish they had known sooner.

A sudden change in behaviour can sometimes be caused by an underlying health issue such as a urinary tract infection (UTI), dehydration, constipation, an infection or even a reaction to medication.

Recognising these changes early and seeking medical advice promptly can make a real difference.

"She's settled in." But what does that actually mean?

When families are told their loved one has "settled in", it often brings a sense of relief. It sounds reassuring, but it's also a phrase worth understanding.

Sometimes settling in simply means someone has become familiar with new surroundings, routines and carers. However, in some situations, it may also coincide with changes to medication, behaviour or daily routines. As a family member, it's completely reasonable to ask questions and understand exactly what has changed and why.

If your loved one has dementia or Alzheimer's disease, don't be afraid to ask:

✔️ Has any medication recently been started or changed?
✔️ Why was that decision made?
✔️ Has their GP or specialist reviewed it?
✔️ What benefits are you expecting to see?
✔️ When will the medication be reviewed again?

If you signed your house over to your kids to protect it from care home fees, I have bad news. The council can undo it.

Everyone trusts the seven year rule. For care fees, that rule does not exist. The council can look back as far as they want, ten, fifteen, even twenty years. If they decide you did it to avoid care costs, they treat you as if you still own it, refuse to fund your care, and can even chase your children for the money.

So before you gift a home or savings to protect them, get proper advice. Getting it wrong can cost your family everything.

The government made an unpaid carer pay back over £4,000. Her crime? Earning one penny too much.

Carer's Allowance pays about £2.50 an hour, and going a single pound over the limit costs you the entire payment. Then they demand back every penny they say you owe.

Over 136,000 carers are now trapped in this debt, a quarter of a billion pounds. And the government had the data to warn people, and did not.

If you claim Carer's Allowance, check your earnings against the limit yourself. Never assume they will tell you.

Complain about your mum's care home, and they can throw her out. It is called a revenge eviction, and it is more common than you think.

You raise a concern, unexplained bruises, missed medication. Days later, a letter arrives asking her to leave.

The message is simple. Stay quiet, or she is gone.

But here is what they are betting on. That you do not know your rights.

They cannot lawfully evict her just for complaining, and they cannot simply remove her.

She is owed proper written notice, usually 28 days, and you can challenge it and report the home.

Never let a home silence you. Complaining is your right.

If you only ever see a care home on a booked tour, you are watching a performance. Not the real thing.
Every home looks wonderful when it knows you are coming. So turn up unannounced, at 5pm on a Sunday. The lowest staffing of the week.

Are the call bells being answered, or ringing into the void? Are the residents chatting and engaged, or just parked in front of a television?

That one quiet, unplanned hour shows you how the home really runs when nobody is trying to impress you. Which is exactly how it will run for your mum every other day.

Book the tour if you like. Then go back when they are not expecting you. That is the visit that tells the truth.

When your mum is in hospital, you can be pushed into a decision that changes her whole life. And most families have no idea they are allowed to say no.

A hospital is the worst place to judge what someone can manage long term. People are at their lowest there, confused and dependent. Decide on the ward, and you almost always overestimate how much care they will need forever.

It should be the other way round. Get her home, or somewhere calm, let her recover, then assess what she actually needs. There is even a name for it, discharge to assess. But in the panic, families get talked into permanent decisions on day one.

So if that pressure comes, slow it down. Ask for a proper assessment once she has recovered, not on the ward. You are allowed to say that.
