. During this time I held an Honorary contract with Nottinghamshire Healthcare Trust at Queens Medical Centre, treating people with complex mental health problems. CBT CAREER GOAL CHALLENGE

As you know, career development for CBT therapists is one of my passions. The journey from deciding on this career right through to establishing a satisfying position after qualifying.

I believe that career development is overlooked by organisations and higher education institutions so it is not valued by individuals either. I do recognise that it is not a 'sexy' topic but nonetheless it affects us all. There is nothing worse than feeling stuck and going through the motions.

I wrote THE CBT CAREER GUIDE to celebrate, motivate and give hope that your career is something you can, and need to, take control of as the truth of the matter is that no one else will!

In one of the chapters in the book 'Having a Career Vision' I suggest creating what I call a CBT career bucket list. Yes, this may sound dramatic but that is the idea. We need to dream and imagine a satisfying future first, and then plan the steps to achieve it no matter how long it takes.

This challenge is about taking a SMALL step towards one of your goals. It may be doing an online search for local courses, or networking to learn what support communities exist around you, or it may be contacting a publisher with an idea for a book.

FORMULATION TIP

I have shared a lot on formulation as I absolutely love it! Sometime we can become set in our way of formulating. Maintenance cycle is always the first step but sometimes we may not go on to develop a full longitudinal formulation. Of course there are many factors that influence our choice here. You may prefer to work at the behavioural level in which case a longitudinal formulation may not be considered necessary.

Regardless of your approach, if you do see formulating beliefs at the core belief and rule levels useful to the treatment for an individual then you may consider identifying some KEY cognitions. In the diagram below I have identified some core beliefs in a cognitive triad then at the centre linked it to a key rule for living.

This maintains theoretical fidelity but can simplify the process for both you and the client.

USING FORMULATION IN SUPERVISION

It won't surprise you that I promote the use of formulation at every opportunity as it is such a robust and powerful means for working through any challenges in therapy. And that applies to supervision too.

This is an anonymised but real example of a therapist exploring their supervision experience using a vicious cycle. The cycle used is embellished with some cognitive processes to show the impact of past experiences and therapist schema (in this example relating to unrelenting standards) played out through rumination.

This can then be used to do some experiential work in session. For example, comparing feelings before and after talking openly about the risky client. I have always found the CBT from the inside out approach so useful as a learning tool.

This cycle led to the following learning points in the session:

It’s not nice to experience this
it puts me into fight/flight/freeze
it prevents me from experiencing the positive information
it’s not fair that I am going through this
it’s time to update this
we can learn to respond to it differently
I’ve done my best
these things are out of my control
I am not solely responsible for the client’s recovery
my colleagues praise me for my work
I get positive feedback from clients and everyone
I give my compassion to others
I AM GOOD AT MY JOB
I deserve to feel proud of myself

Had a fabulous time last night with the UK Therapy Guild trainees in Liverpool. This was the final of 3 sessions introducing CBT. It was lovely to see threat turn to curiosity as the sessions progressed.

This counselling course headed by my cousin Nichola Doyle is incredible as it provides an opportunity for people from all backgrounds to commence their therapy career journey in a supportive and inspiring environment. Considering that CBT is not their primary modality they fully immersed themselves in the learning and activities. The group discussions and collective work produced was of a very high standard for the stage in their journey and was great to be witness to.

Well the last 24 hours didn’t go quite as planned! This seems to be the norm for me recently since my America trip! Definitely threat system overload which makes my brain way too chaotic! I can normally function well and used to ā€˜run on adrenaline’ as I used to say!! Haha! How ironic! Well, I am realising something as I mature - adrenaline is not always your friend, especially the older you get!

So yesterday’s and today’s original plans:

Thursday
10-11am client session

6-9pm teaching session in Liverpool (110 miles away)

Friday
CPD all day event in Stirling Scotland

Spot any challenges??! Well my inattentive brain hadn’t factored in the following things because I am completely TIME BLIND!

- after client session I still needed to prepare handout packs x 26. Ran out of paper and toner so had to shop for that.
- hadn’t prepared my campervan or packed
- hadn’t eaten
- hit rush near Liverpool making journey longer
- thought Stirling was closer to Liverpool!

So this is what actually happened.

After rushing chaotically, then slow traffic, I arrived at Liverpool just 10 minutes before session was due to start.

Had brilliant session which put me on a giddy high, which likely caused me to lock my keys and bag in the van. Caretaker rang my cousin and daughter who rang around for a locksmith, but in my panic to get the door open I broke the handle!! They were true angels!

At nearly 11pm and £95 down I realised that Stirling was actually 240 miles away (4 hours 20 mins).

Drove over 200 miles but had to crash (at a service station without paying!) and missed the training!

Have sent apologies and I’m off to Loch Lomond! šŸ«£šŸ˜„šŸ˜

I know many people think ADHD is a trend or an attention seeking ploy but the struggle is real! (said in a totally calm and non-defensive tone!!ā¤ļø)

Quick update - I’ve put my keys on a lanyard! A pretty one too!! There’s always a lesson to be learned in these situations! And bling always helps!! šŸ˜†
