From the 1st of April 2026 we will be calling you in for your 'annual review' by your birth month. This will make it easier for you to remember when you are due a review for the following conditions, and also means we can do it all in one appointment:
asthma
copd
dementia
diabetes
annual bloods
heart failure reviews
mental health reviews
Anything that is not listed above will be called in routinely when due.
