There is a persistent crisis in hospital discharges for autistic people and those with a learning disability. As of August 2025, there are approximately 2,025 autistic people and people with a learning disability in mental health hospitals.

Preventing delayed hospital discharges requires more than isolated fixes. It demands a joined-up, system-wide approach that addresses the root causes of delay. At its core, this means strengthening three critical areas: coordination and communication across services, robust home care provision, and effective transitional care programmes.

That is why we developed our Bridging Support model.

We have designed this model intentionally to create the right conditions for people to move forward and remain in their communities.

“It’s what happens when we get the fundamentals right: trust, choice, compassion, and presence.” 💟

For years, Elliot’s life was shaped by restriction and routine that wasn’t his own.

Today, his days look very different.

From choosing how he spends his time, to ordering his own hot chocolate and sitting down to enjoy it, these moments might seem small, but they represent something much bigger: independence, confidence, and control over his own life.

With the right support around him, Elliot isn’t just being cared for, he’s living life on his terms. Building routines that work for him. Making choices that matter. And reconnecting with the world in a way that feels safe and meaningful.

Really listening to William changed everything.

Not just hearing what was difficult, but understanding what he loves. Word games. Trains. Time outdoors. The things that make him who he is.

When a consistent team came into his life, people who took time to genuinely know him, small things began to shift.

A first train journey, with William smiling the whole way.

A family holiday in Scotland.

Days that now include calm, laughter, and things a 20-year-old should get to enjoy.

Much of this work happens behind the scene, from responding to urgent referrals, to working with internal teams to mobilise support, to making sure communication stays clear and transparent for everyone involved.
