Are you an experienced care professional looking for an opportunity to take the next step in your career? 💙

Excel Care Group is looking for a Care Coordinator to join our team on a maternity cover basis.

There is a possibility this position could become permanent.

📍 Office & Community Based
🕣 Monday – Friday | 8:30am – 4:30pm
🚗 Must be a driver with your own vehicle
📞 On-call rota – approximately 1 week in 3 (paid)
🏡 Community sickness cover when required

What will you be doing?

You’ll play an important role in coordinating our care services, supporting our care teams and ensuring our clients receive safe, consistent and high-quality care.

The role will include:
✔️ Coordinating staff rotas and arranging cover
✔️ Supporting staff with day-to-day queries
✔️ Liaising with clients, families and professionals
✔️ Managing sickness and absence cover
✔️ Supporting on-call
✔️ Visiting the community and providing care cover when required
✔️ Monitoring care packages and staff allocation
✔️ Maintaining accurate records and care documentation
✔️ Escalating concerns and supporting safe service delivery
Previous Care Coordinator experience is desirable but not essential — full training and support will be provided.

🎓 Training is due to commence as soon as possible so we are looking for someone who can commit to starting their training immediately.

💷 Pay: up to £15 per hour
🕒 Job Types: Full-time | Temporary to Permanent

✨ About the Role
You’ll provide 1-2-1 care in clients’ homes, supporting individuals with complex clinical needs.

This includes:
• Tracheostomy care & ventilation support
• Medication management & clinical observations
• PEG feeding & nutritional support
• Personal care with dignity and respect
• Working closely with families and healthcare professionals

👩‍⚕️ What Makes This Role Special?
This isn’t standard home care you’ll support clients with:
• Long-term ventilation needs
• Neurological conditions
• Palliative & end-of-life care
• Complex physical disabilities

📈 What We Offer
• Specialist clinical skills training
• Ongoing development & career progression
• 24/7 clinical on-call support
• Regular supervision & competency assessments
• Wellbeing & mental health support

✅ Requirements
