📢 Important Update – New Way to Access Care

From Monday all appointment and admin requests will go through our online Patient Triage form.
You can access it:
✅ On our website - https://accurx.nhs.uk/patient-initiated/p84669
✅ Through the NHS App
✅ By calling us – if you can’t complete the form, our reception team will help you complete the form.

Forms are open and available from 8am through to 6pm Monday to Friday.

⏰ Same-day urgent requests (including home visits) must be submitted by 11 am.
After this cut-off you can still request routine/non-urgent appointments or submit admin queries.

Every request is reviewed by a GP/Navigation team the same day. You may be:
👩‍⚕️ Booked with the GP
💊 Referred to another clinician such as pharmacist, physio or mental health worker
📲 Sent self-care advice

If we reach safe capacity for the day or you submit an urgent request after the cut-off, you may be directed to NHS 111 or a local Walk-In Centre.

💊 Medication requests should continue via the NHS App.

We’re making this change to make access safer, faster and fairer for all patients. Thank you for supporting us as we move to Total Triage.
