Derbyshire Dales Primary Care Network (DDPCN) is reaching out to patients aged 30 to 65 with a short questionnaire about family history and cancer risk. If you receive one please take a few minutes to complete it.

Cancer that runs in families can sometimes be linked to inherited genes. By understanding your family history, we can identify whether you might benefit from additional health checks or genetic testing.

The questionnaire takes just a few minutes to complete online using your mobile phone. Receiving it does not mean there is any concern about your health, its simply a routine proactive check being offered to patients across the Derbyshire Dales.

A few answers today could make a big difference tomorrow. 💚

📢 Meningitis B (MenB) Vaccination – Important Update for Young People

A new time-limited MenB vaccination programme has been announced following recent outbreaks.

✅ Who is eligible?

• Under 25s starting university for the first time
• Those moving into student accommodation or halls in Autumn 2026

💉 Where to get vaccinated

👉 This vaccine is NOT available at the GP surgery
👉 Eligible patients should attend a community pharmacy for their vaccination

📅 Vaccinations begin from 20 July 2026.
Patients will be invited to book or can check the NHS website for participating pharmacies.

⚠️ The vaccine offers good protection, but it does not prevent all types of meningitis—please remain aware of symptoms and seek urgent medical advice if needed.

📝 Do you have a Lasting Power of Attorney (LPA)?

A Lasting Power of Attorney (LPA) is a legal document that lets you choose someone you trust to make decisions on your behalf if you’re ever unable to do so.

There are two types of LPA, but for healthcare, the most important is Health and Welfare. This allows your chosen person (your “attorney”) to make decisions about things like:

• Your medical care
• Daily routine (e.g. eating, washing, dressing)
• Moving into a care home
• Life-sustaining treatment

A Health and Welfare LPA can only be used if you lose the ability to make decisions for yourself.

Why is this important?
We want to make sure that, if needed, we know who can speak and make decisions on your behalf. This helps ensure you receive the right care, in line with your wishes.

Please let us know if:
• You already have an LPA in place
• You are an attorney for someone registered with us
Sharing this information helps us keep your records accurate and ensures everyone involved in your care is aware.

We may ask for a copy of the LPA to hold securely on your record.

Derbyshire’s local weight management services have updated their referral pathways to make it easier than ever to get the right help, at the right time!

Derbyshire residents → Live Life Better

Derbyshire resident or registered with a Derbyshire GP
•BMI of 25 or above or 23 if client is of Black/Asian ethnicity
•Aged 18 or over (16–17-year-olds will be supported if referred by a medical professional)

QUEST (Staff Training) Wednesday 10th June Afternoon - Closed for staff training:

On ten Wednesday afternoons per year (from April to March) the practice is closed to provide important routine staff training.

Routine appointments, prescription services and online consultations are closed during these times.

For urgent medical advice during this time, please telephone 111 (DHU-the out of hours service). The Practice contracts/funds 111 to support patients medical queries during these periods.

Darley Dale: closes from 13:00pm until 18:30pm and will re open at 18:30 for our evening clinic.
Winster: closes from 12:00pm
Youlgreave: closes from 12:30pm

The practice will be closed Monday 25th May . We will re-open at 8:00 in Tuesday 26th May.
