ℹ️ This is a typical presentation we see with an acute flare up of calcific tendinitis of the shoulder.

Often comes on veey quickly over a few days where people go from full normal movement to intense pain and massive restrictions in movement.

The lack of movement in an acute flare up often looks similar to a frozen shoulder but the main differential is how quickly it comes on. A frozen shoulder tends to stiffen alot slower over weeks and months not hours and days.

The pain usually goes quite high that alot end up at A+E due to the high pain levels. They x-ray it and often find these calcium deposits. Then they usually end up with us for a corticosteriod injection or barbotage to get the pain under control as nothing else really touches it.

So a good rule of thumb to remember is that quick onset of pain and stiffness that looks like a frozen shoulder but has come on rapidly should lead you to thinking of an acute flare up of calcific tendinits as a differential diagnosis

Not complicated just some hands on controlled passive movements to help get this stiff knee moving.

ℹ️ Patient has had partial ACL tear, MCL sprain and Medial meniscus tear- horizontal tear. Was put in a brace for 6weeks to help MCL sprain.

Has been unable to bend or straighten knee for 4 months in total now.

Seen NHS physio and goven exercises but feels minimal improvement with them, so we decided to get hands on and stuck in!

ℹ️ Sometimes you just need to get hands on to get things moving.

Jan' 25- Patient got hit by dog on outside of knee and pushed knee in the way (valgus).

Fast forward- X-Rays and MRI done, partial ACL tear, horozontal meniscus tesr and MCL tear. Braced for 6 weeks for MCL tear. Been out of it now a while but movement hasnt changed. Has been getting seen on NHS and given exercises but doesnt feel her knee movmemrt has changed much in 4 months. (Cant fully straighten yet, cant bend past 30-40deg).

As there is no mechanical block on a scan by the structures invovled, I felt like the restriction is most likely a mix between gaurding, and capsular and soft tissue contraction due to minimising movment because of pain at the start and the initial bracing.

I did a mix of simple passive flexing, rotation and extension with a bit of distraction, with some isometric contractions. And you csn see how quickly it changes.
