When I first came across this quote, I felt genuinely surprised and incredibly humbled...

If someone as accomplished as Maya Angelou could feel like that, perhaps thoughts of imposter syndrome are not a sign that we are not good enough after all.

As a doctor, I still have moments where I question myself. Despite years of training, experience, caring for thousands of patients, and receiving such positive feedback, that little voice can still appear from time to time.

And I see the same thing in so many of the colleagues I work with, as well as the patients I have the privilege of caring for.

These are women who are exceptional at their jobs. Women who are wonderful mothers.
Women who are running businesses, caring for ageing parents and supporting everyone around them...

Yet, quietly wondering whether they are doing enough, or whether they are somehow going to be "found out."

As a women's health specialist, I also wonder whether these feelings can become even more pronounced during times of significant hormonal change, such as the postpartum period and perimenopause. We know these life stages can affect mood, anxiety, sleep and cognitive function, and many women describe feeling less confident, experiencing brain fog, or questioning themselves more than ever before.

The reality is that self-doubt does not always reflect ability. Sometimes it simply reflects just how much you care about what you do.

✨️ MHRA Licence Extension for Elinzanetant (Lynkuet): A Significant Step Forward in Menopause Care After Breast Cancer ✨️

For many women with breast cancer, hot flushes and night sweats are far more than an inconvenience. They can be relentless and exhausting, disrupting sleep, affecting concentration, and having a profound impact on quality of life.

These symptoms often occur as a result of endocrine therapies, such as tamoxifen or aromatase inhibitors (e.g. letrozole), or because chemotherapy or radiotherapy has induced menopause by affecting ovarian function.

Until now, treatment options for breast cancer survivors experiencing menopausal symptoms have been limited. Hormone replacement therapy (HRT) is generally not recommended for most women with hormone-sensitive breast cancer, although this is a nuanced discussion and treatment decisions should always be individualised.

That is why the MHRA’s extension of the licence for elinzanetant (Lynkuet) to include women with breast cancer is such encouraging news.

Elinzanetant is a non-hormonal treatment that works by blocking neurokinin receptors (NK-1 and NK-3) in the brain, helping to reduce hot flushes and night sweats without the use of oestrogen.

It is now licensed for the treatment of moderate to severe vasomotor symptoms associated with menopause, including those caused by endocrine therapy in women with hormone receptor-positive breast cancer.

The phase 3 OASIS-4 clinical trial showed that elinzanetant significantly reduced the frequency and severity of hot flushes compared with placebo.

These benefits are particularly important because troublesome menopausal symptoms can lead some women to stop taking their endocrine therapy prematurely, which may affect the long-term effectiveness of their breast cancer treatment.

However, as with any treatment, it is important to discuss your individual circumstances and carefully weigh up the potential benefits and risks with your healthcare professional.

At present, elinzanetant is only available via private prescription. The next challenge is ensuring that all eligible women can access this treatment through the NHS.

In England, NICE is currently appraising the clinical and cost-effectiveness of elinzanetant for moderate to severe vasomotor symptoms associated with menopause, which will help determine future NHS availability.

This represents an important step forward in improving the quality of life for women affected by breast cancer.

Every woman deserves not only to survive cancer, but to live well afterwards.

✨ Bioidentical HRT vs Body identical HRT: Are they the same thing? ✨

At first glance, they sound like the same thing, and the terms are often used interchangeably. In fact, both contain hormones that are structurally identical to those produced naturally by the human body.

However, there are some important differences that every woman should understand so that she can make an informed choice about her treatment.

🧪 BIOIDENTICAL HRT (also known as compounded bioidentical HRT or cBHRT)

• Hormones that are prepared by specialist compounding pharmacies.
• Not licensed or regulated by the MHRA.
• Not routinely recommended by the British Menopause Society due to concerns about quality, consistency, efficacy and safety.
• Not available on the NHS.

🩺 BODY-IDENTICAL HRT (also known as regulated bioidentical HRT or rBHRT)

• Hormones that are manufactured by pharmaceutical companies.
• Produced to strict quality standards with consistent dosing.
• Licensed and regulated by the MHRA.
• Recommended by the British Menopause Society and NICE where appropriate.
• Widely available on the NHS.

I often see adverts on social media promoting compounded bioidentical HRT. These treatments can be very expensive and are sometimes marketed as being more "natural", more personalised, or even superior to the HRT prescribed on the NHS.

What is often not explained is that the regulated 17β-oestradiol and micronised progesterone available on the NHS are also body-identical (bioidentical) hormones. In addition, there are also synthetic hormone options available where clinically appropriate.

Regulated bioidentical HRT can also be highly individualised. Treatment can be tailored by adjusting the hormone type, dose and route of administration (for example, patches, gels, sprays, tablets and vaginal pessaries), allowing treatment to be personalised according to each woman's symptoms, medical history, preferences and treatment goals.

The terminology can be confusing, but it's important to know that "natural" does not necessarily mean compounded, and personalised care does not require compounded hormones.

Some consultations stay with you long after the clinic has finished…

Recently, I had the privilege of caring for one of my oldest patients yet. 🥰

A relative contacted EveryWoman Health because she was deeply worried about her. For many years, she had been living with worsening bladder, vaginal and vulval symptoms that had become so overwhelming she no longer felt life was worth living.

My heart broke for her, not because her condition was untreatable, but because she had suffered in silence for so long, believing this was simply the price of having children and getting older.

Years earlier, attempts had been made to help. A vaginal ring pessary to support her bladder prolapse had been unbearably painful and traumatic, and vaginal oestrogen pessaries were stopped because she struggled to insert them due to poor hand dexterity.

So she carried on suffering…

When I examined her, the cause of her pain was clear. Her vulval, vaginal and urethral tissues were extremely thin, fragile and inflamed due to untreated genitourinary syndrome of menopause (GSM). She also had a small bladder prolapse.

Before considering treatment for the prolapse, our first priority was to restore the health of her tissues and relieve her discomfort.

Together, we chose a vaginal oestrogen cream that she could use more easily and consistently. Vaginal oestrogen is a very low dose treatment with minimal absorption into the bloodstream and is safe and effective for most women, regardless of age.

Guided pelvic health physiotherapy is often recommended alongside vaginal oestrogen to help to strengthen the pelvic floor and help improve prolapse symptoms. However, because of her frailty, this was not a realistic option.

Our goal was simple: to help her tissues heal, improve her comfort and restore some of the quality of life she had slowly lost.

This story highlights something I see far too often. Many women avoid examinations, cervical screening and gynaecological care because they have become too painful. Treating the underlying cause first can often make these experiences much more comfortable and possible.

No woman should believe she simply has to endure symptoms because of her age.

Whether you are 50, 70 or in your 90s, you deserve to be listened to, supported and offered treatment that is right for you.

You deserve comfort. You deserve care. You deserve hope.

I was back in London today for a conference and could not resist taking a quick trip down memory lane...

It happened to be King's College London's open day, and the campus was full of bright-eyed, enthusiastic prospective students. It was so lovely to see the excitement, optimism and ambition on display. Observing them brought back memories of being in exactly the same position myself and believing that graduation would be the final destination.

I graduated from King's College London in 2012, and I am incredibly proud of that achievement and the many happy memories associated with it. Looking back, though, I have come to realise that graduation was not the end point — it was just the beginning.

The real education came afterwards: through patients, clinical experience, challenges, personal growth and life itself.

Fourteen years on, I am still learning.
