The concentration says it all.

Taken during our End of Life Care training while practising the preparation of anticipatory medicines.

There is a lot resting on a nurse’s shoulders at this point. Getting the right medicine, the right dose, the right concentration, the right compatibility and the right volume, while knowing that the person receiving it may be in the final hours or days of their life.

This is why we practise for real during our training, because when the time comes to use these medicines, that nurse needs to be confident, competent and able to get things right.

Yesterday (yep, on a Saturday.!!) we delivered one of the most fascinating and thought provoking courses we offer.

What happens in our last hours of life and what happens to the body after death

Delivered by our specialist trainer, a former mortician with many years of experience.

We explored the physical changes that occur during a person's final hours, and the minutes after death, as the body begins to cool and rigor mortis develops.

We heard about the many logistical challenges undertakers face when removing a dead body from a care home, manoeuvring stretchers through narrow corridors, around tight corners and into lifts that were never designed with this in mind. You'd be amazed at the many tips and tricks they have up their sleeves to ensure a dignified final journey.

We looked at what actually happens during post mortem, why such a referral may be required and how post mortem can sometimes uncover diseases or medical conditions that were never diagnosed during a person's lifetime.

We discussed how, once all examinations have been completed, the person is carefully prepared before being released into the care of the funeral director. This may include washing, dressing, hairstyling and the careful application of natural looking cosmetics, helping loved ones say their final goodbye in peaceful surroundings.

And to top it all off, our trainer introduced us to a brand new word: . Yep, that's what we're all called once we've died.!

The feedback was outstanding, with several people describing it as one of the most interesting and memorable training sessions they had ever attended.

Death is something every one of us will encounter, both personally and professionally. The more we understand it, the better equipped we are to care for others with confidence, compassion and dignity.

This week, my team met Elvis.!!!!!

Not only that, they were treated to scrummy 99 ice creams while chatting with residents and staff.

Every care home has its own personality. Some have incredible gardens, some have amazing activities, and occasionally... they have Elvis and ice cream.! ❀

Thank you. The ice cream was very much appreciated, especially in this weather.

Our training room looked a little different last week...

Thanks to the extreme heat, our Medication Competency Assessment moved outside under the shade of a tree.

No PowerPoint. No air conditioning, just medication charts, practical discussions and a group determined not to melt before lunchtime. đŸ„”

High temperatures affect concentration, fatigue and wellbeing. If we're asking staff to demonstrate safe medication practice, we need to create an environment where they can perform at their best. Sometimes that means adapting.
...And in the end, the training worked brilliantly.!

They’re open to anyone working in health and social care, whether that’s care managers, care staff, nurses, or hospice teams.

We design the sessions to suit you, so they can be delivered just for managers, just for care staff, or as a mixed group.

The subjects we cover are shaped around what we know our clients are actually dealing with day to day.

Subjects we have delivered recently includes areas like medication management and governance, safeguarding, MCA and consent, moving and handling, dementia and distressed behaviour, dignity at work, communication, and clinical skills such as PEG feeding, epilepsy care, suctioning, and oxygen therapy.

Venepuncture.

It doesn't look like much, a needle, a tourniquet, a few tubes...and a real human in front of you, trusting you to get this right first time.

Taking blood is not just about getting into a vein. It is knowing which vein and where to go, what you are feeling under your fingers, and when something does not feel right. It is also about avoiding unnecessary pain, preventing bruising, and knowing when to stop rather than pushing on and causing harm.

I can always tell when someone has only learned this online. They know the steps, but not the feel of it. They are not quite sure what they are looking for. That's where things go wrong.

This is why hands on training is so important.You need to feel the difference between a good vein and a poor one. You need to see what happens when things do not go to plan. You need someone there to guide you, correct you, and build your confidence properly.

If your team is expected to carry out venepuncture, make sure they are properly trained, not just signed off on paper.

This is part of a PEG feeding set from a real person.

Simple equipment, but behind it sits a person who cannot eat or drink safely in the usual way. For them, this tube is not optional. It is how they receive nutrition, hydration, and often their medication.

PEG care is not just about connecting a syringe. It is about preventing infection, recognising complications early, protecting the stoma site, and understanding what normal looks like so you can spot when something is wrong.

When staff understand this properly, PEG feeding becomes safe, calm, and routine. When they do not, the risks increase quickly.

This is why proper, hands on training is important.
