New MY CARE app for care carried out at UHP Derriford.

MY CARE allows you to view:
✅ Hospital test results
✅ Appointment information
✅ Details about your hospital and specialist community care
✅ Information for family members or people you care for (where appropriate proxy access has been arranged)

MY CARE works alongside the NHS App, which continues to provide access to your GP records, repeat prescriptions, vaccinations and NHS services.

⚠️ Important: If you have questions about a hospital test result or information shown in MY CARE, please contact the hospital team that requested the test or is providing your care.

Hospital clinicians remain responsible for reviewing, interpreting and acting on hospital-ordered results. GP surgeries are not responsible for interpreting or following up these results and may not have the information needed to answer your query.

📊 August at Peverell Park Surgery – By the Numbers

August was another busy month, with our team supporting patients through both our telephone and online services.

📞 5,880 incoming calls received
📞 5,657 outbound calls made
💻 5,313 SystmConnect requests submitted

The most common online requests were:
🩺 New medical problems: 1,730
💊 Medication requests: 1,425
📋 Existing condition reviews: 1,052

Our busiest day was Monday, with the busiest period between 8am and 9am.

There is still a common misconception that you need to call before 9am on a Monday to get help. In reality, we review and respond to online consultations throughout the day, and our phone lines remain open to support patients whenever they need us.

There have been around 14 million consultations through the pharmacy first service since it launched in February 2024.

You can get treatment for these conditions from your local community pharmacist, including some prescription medicine, if needed – without you seeing a GP.

From this autumn, participating pharmacies will be able to assess and treat 5 additional common conditions:

- migraine in adults
- outer ear infections in adults
- acne in young people and adults
- mild skin infections and scabies
- seasonal allergic rhinitis in children
