Menopause, Brain Health and Alzheimer's Disease: What Does the Latest Research Tell Us?
As our understanding of women's brain health continues to evolve, researchers are paying increasing attention to the relationship between menopause and Alzheimer's disease. While many questions remain, several important themes are beginning to emerge from the evidence.
Firstly, it is important to understand that menopause does not directly cause Alzheimer's disease.
What researchers are increasingly discovering, however, is that the hormonal changes that occur during midlife may influence how the brain ages. As estrogen levels decline, changes occur in brain energy metabolism, neural communication, inflammation regulation, and cognitive function. These biological shifts may affect an individual's vulnerability to neurodegenerative diseases later in life.
One of the most closely studied areas is the role of estrogen-based hormone replacement therapy (HRT). Recent findings have suggested that women using estrogen-only hormone therapy may have lower rates of Alzheimer's-related brain pathology and dementia diagnoses compared with non-users. These results have renewed interest in the potential neuroprotective effects of estrogen and its role in maintaining brain health after menopause.
Current studies show an association, not proof that estrogen directly prevents Alzheimer's disease. Researchers continue to investigate the importance of factors such as treatment timing, hormone formulation, duration of use, and individual patient characteristics.
For this reason, HRT is not currently recommended solely as a strategy to prevent Alzheimer's disease. Larger randomized clinical trials are still needed.
The emerging consensus is not that menopause causes Alzheimer's, but that menopause may represent an important window in women's brain health. Understanding how hormonal changes influence brain ageing could help shape future approaches to dementia prevention, earlier intervention, and personalised care for women throughout midlife and beyond.
