“I just look tired.”
It’s something I hear surprisingly often in clinic and it doesn’t necessarily mean someone wants to look younger.

Sometimes changes in skin quality, facial volume and the way light and shadow fall across the face can leave us looking more tired than we actually feel.

That’s why I don’t start with a treatment. I start by working out what’s creating that impression, what can realistically be improved, and what should be left alone.

Because the goal isn’t to change your face.
It’s to help your reflection feel a little more like you again.

This is me, off duty.
No makeup. Hair dried naturally. In my comfies.
And this is actually the goal for me.

Not flawless skin. Not looking 10 years younger. Not constantly finding something else to “fix”.
I still have treatments. I still wear makeup. I still care about how I look.

But I’m much more interested in feeling comfortable in my own skin than chasing perfection.
Because good aesthetics, to me, should support confidence, not create a never-ending list of things to improve.

I’ve been thinking a lot about the way skin is perceived.

Research suggests that more even-looking skin is associated with being seen as healthier, younger and more attractive.

But healthy-looking skin doesn’t happen in a vacuum.

It can reflect access to time, money, knowledge, treatments, healthcare and consistency.

And that’s where it gets interesting.

The modern luxury aesthetic often looks effortless — bare skin, minimal makeup, healthy hair, white teeth, a rested face.

But “effortless” can require an enormous amount of effort.

Perhaps clear, glowing skin has become one of the ways we quietly communicate access and affluence.

That doesn’t make it wrong to invest in your skin.

It just means we should be careful not to confuse looking well with being worth more.

Lip filler has a reputation problem… and I can understand why.

Before I worked in aesthetics, when someone said “lip filler”, I pictured the same thing many people still do: very large, obviously augmented lips.

And I knew that wasn’t for me.

What I quickly learnt, though, is that hardly anyone who sits in my chair actually asks for that look.

Most patients say:

“I don’t want big lips.”
“I still want to look like me.”
“I don’t want people to know I’ve had them done.”

And those are exactly the lips I like treating.

For me, lip filler isn’t about putting the same shape or the same volume into every pair of lips. It’s about looking at the lips in front of me, their anatomy, proportions, existing shape and what has changed with age — and deciding what, if anything, would benefit from a little support.

Sometimes that means restoring lost volume.
Sometimes it’s improving definition or balance.
Sometimes it’s simply giving the lips a fresher, softer appearance.

The goal isn’t for someone to look at you and think “nice filler.”

It’s for them to think “she looks good” without being quite sure why.

So if you’ve always said “I hate lip filler”…

Perhaps what you actually hate is obvious lip filler.

A few things I believe about aesthetics.

More isn’t always better. Trends aren’t treatment plans. And my job isn’t to convince you that something needs “fixing”.

I believe in good skin, subtle changes, informed choices and knowing when not to treat.

You can love yourself and still choose aesthetic treatments. The two were never mutually exclusive.

Ultimately, I want you to look like you, not a trend.

Where does self-care end and vanity begin?

I don’t think there’s a simple answer.

Most of us alter our appearance in some way. Hair colour, makeup, skincare, Botox, filler, surgery, we just draw the line in different places.

And those choices aren’t made in a vacuum. Culture, beauty standards and the people around us all influence what feels “normal”.

I know that personally too. I work in aesthetics, I’ve had Botox and filler, and I prefer how I look because of some of those choices.

So I’m not interested in judging where someone sits on the spectrum.

I’m more interested in whether the choice feels informed, comfortable and genuinely theirs.

Male profile balancing ✨

The aim with Jacob wasn’t to change his face, it was to subtly enhance structure, proportion and balance.

Filler was strategically placed through the lateral zygoma, anterior cheek, gonial angle and chin, maintaining masculine proportions while enhancing cheek projection and lower-face definition.

The result is deliberately subtle. Still completely Jacob, just more balanced.

Assessment first. Treatment plan second. Filler only where it adds value.

A little more volume. A little more hydration. Still her.

Lip filler doesn’t have to be about creating a completely different lip shape or chasing a particular aesthetic.

For this patient, 0.7ml Teoxane RHA Kiss was used to add subtle volume while keeping the result proportionate to her existing features.

Nothing needed fixing. She simply preferred a little more.

And I think there’s an important difference.

The more I scroll through social media, the more I notice it: everyone is starting to look the same.

Tiny noses. Full lips. Defined jaws. Smooth skin. Perfect symmetry.

But our differences are part of what makes us recognisably human.

I’m obviously not anti-aesthetics 😂 But I am against making normal human variation feel like something that needs correcting.

For me, good aesthetics should help you look healthy, rested and well, without erasing the features that make you you.

I don’t want my patients to look perfect.
I want them to look like themselves. Just really bloody well.

Today marks another important milestone for Aesthetics by Carly.

I’m proud to have been accepted onto the Joint Council for Cosmetic Practitioners (JCCP) Register. JCCP-Certificate.pdf

The JCCP is the UK’s professional register for aesthetic practitioners, established to promote high standards, clinical governance and patient safety.

Registration is voluntary, but I believe choosing to go beyond the minimum expected matters.

Alongside being a Registered Nurse Independent Prescriber, Save Face Accredited clinic and practitioner, this registration reflects my ongoing commitment to practising safely, ethically and in line with the highest professional standards.

When you choose a practitioner, you’re placing your trust in them. That trust should be earned through more than before and after photos.
