🌟 As part of the QI Leaders Pathway 2025 Vets for Pets course, Tracy Pentelow led a thoughtful quality improvement project focused on clinical documentation and communication.

🐾 Her project, “Anaesthetic Forms - Recording the Finer Details”, explored how reliably anaesthetic and sedation records were being completed, recognising that missing details can affect aftercare, continuity, and patient safety, especially on busy clinical days.

By testing a series of change ideas through PDSA cycles, including form redesign, visual prompts, team engagement, and reminder systems, Tracy gained valuable insight into what does (and doesn’t) lead to sustainable improvement. While some short‑term gains were seen, the project highlighted the complexity of embedding lasting change and the importance of system‑level solutions over reminders alone.

✅ The outcome? Improved understanding of the problem, stronger learning around sustainability, and a clear foundation for future improvement to support safer anaesthetic care and better record‑keeping.

📢 Opportunity: QI Conference 2026 - Call for Abstracts

The Royal College of Psychiatrists is hosting its Quality Improvement Conference 2026 on Monday 12 October in London.

This annual event will spotlight co-produced quality improvement (QI) work in mental health services, with a focus on how teams are embedding QI into everyday practice for long-term impact.

💡 Abstract submissions are now open, with interest in:
Practical, frontline-led innovations
Rapid-cycle testing and real-world solutions
Embedding QI approaches into daily work

🌟 As part of the QI Leaders Pathway 2025 Vets for Pets course, Kelly Gordon led a high‑impact quality improvement project focused on procedural safety.

🐾 Her project, “Paws, Check, Proceed”, aimed to improve completion of pre‑procedure safety checklists - a vital safeguard to prevent omissions, near misses, and potential harm before sedation, anaesthesia, or surgery.

By testing small, system‑focused changes, such as redesigning the checklist layout, making it more visible, clarifying shared responsibility between vets and nurses, and using visual prompts, Kelly achieved a sustained increase to 100% checklist completion.

✅ The result? More reliable safety processes, reduced risk of preventable errors, and a stronger culture of shared accountability for patient safety.
