Intensive short-term mental health support is being offered to older inpatients in Shropshire, Telford and Wrekin to help with their transition back home, thanks to a new partnership.
Midlands Partnership University NHS Foundation Trust (MPFT) is working with charity Landau Education and Supported Employment to provide the 12-month Hospital to Home pilot supporting service users aged 65+ who have been discharged from The Redwoods Centre following an inpatient stay.
The service seeks to make the transition home as seamless as possible for the service user, reducing the potential for a return to hospital and supporting the NHS 10 Year Plan’s key shifts towards community-based care and proactive prevention.
Conversations with the service user will start during their stay at The Redwoods to establish their level of need. Upon discharge, the team will provide practical and emotional home-based support for up to four weeks as required. This may include building their confidence in socialising or helping with relationships.
The work of the team will enhance the support currently provided by MPFT’s Community Mental Health Service and other teams in Shropshire, Telford and Wrekin.
Anton Dias, Service Manager for Urgent Care in Shropshire, Telford and Wrekin said: “We’re really pleased to be working with Landau to offer this additional mental health support to service users aged 65+ who are in the process of being discharged home from The Redwoods Centre.
“When a service user has had a stay in hospital it can be an overwhelming experience returning home, particularly if that support network, such as family members isn’t there. By developing that relationship with them while they are at The Redwoods, we can help to make that transition back home less challenging.
“In offering a four-week period of intensive support, we’re able to help the service user with settling back in at home and working with them to readjust. By enhancing the support we’re able to offer during this critical period we can also help to reduce the risk of a person deteriorating and needing to be readmitted.”

By working alongside MPFT’s clinical teams and providing practical, person-centred support around areas such as housing, finances, confidence and social connection, we can complement the clinical care already in place and help people settle safely back into their communities. Together, we hope to support recovery, independence and wellbeing, while reducing the risk of avoidable readmission.”
