Working alongside the GP's and care navigators to support patient care and signposting

Supporting patient triage and appointment requests

On Thursday 18th June, members of the Threeways Surgery team attended the Community Hub meeting, where they joined representatives from Stoke Poges Parish Council, South Bucks PCN, and members of the local community for an informative and engaging morning. During the session, Ritisha Parmar from Visiting Angels delivered a presentation on falls prevention, sharing practical advice on reducing the risk of falls both at home and whilst out and about. The talk covered simple but effective measures, including improving home safety, staying physically active to maintain strength and balance, and recognising potential hazards, helping attendees gain valuable knowledge to support their own health, wellbeing, and independence.

The Community Hub meetings take place on the third Thursday of every month at Stoke Poges Village Hall. Everyone is welcome to attend, with complimentary tea and coffee provided. It is a great opportunity to meet others in the community, enjoy a friendly chat, and take part in a different health and wellbeing topic each month.

The next Community Hub meeting will take place on Thursday 16th July, from 10:30am to 11:30am. We encourage everyone to come along, meet new people, and take part in another interesting and informative session.

- A very high temperature
- Hot skin without sweating
- Fast breathing and a fast heartbeat
- Confusion and restlessness
- Seizure (fit)
- Loss of consciousness

โ˜€๏ธ Heatwave Advice And How To Stay Safe In Hot Weather โ˜€๏ธ

Hot weather can affect anyone, but some people are at higher risk of becoming unwell, including older adults, babies and young children, pregnant people, those with heart or breathing problems, people who work or exercise outdoors, those experiencing homelessness, people living alone, and those in hot or poorly ventilated homes (such as top-floor flats).

In hot weather, people may develop dehydration, heat exhaustion, or heatstroke. Heatstroke is a medical emergency.

To stay safe:
๐Ÿ’ง Drink plenty of water regularly (donโ€™t wait until you feel thirsty)
๐ŸŒค๏ธ Avoid the sun between 11amโ€“3pm
๐Ÿงด Wear light, loose clothing, a hat, sunglasses, and use sunscreen
๐Ÿ  Keep your home cool by closing blinds/curtains during the day and opening windows when cooler at night
๐Ÿš— Never leave children, vulnerable adults, or pets in parked cars
๐Ÿƒโ€โ™‚๏ธ Plan exercise or activity for cooler parts of the day
๐Ÿ“ฑ Check weather alerts and follow local heat health guidance

Look out for signs of heat exhaustion: dizziness, headache, nausea, heavy sweating, and weakness.
If symptoms worsen or confusion/high temperature occurs, seek urgent medical help.

Please check on friends, family, and neighbours who may be vulnerable.
